WESTERN PUMP <br /> Petroleum & Lubrication Equipment Specialists <br /> On occasion , an employer and a discharged employee may sign a "rehabilitation agreement" or a "last <br /> chance" agreement, under which the employer voluntarily reinstates the employee in return for a <br /> pledge to enter treatment as well as to improve job performance . Usually, the employee agrees that <br /> should he/she violate the terms of the agreement, either by failing to remain in treatment or by further <br /> misconduct at work, he/she will be terminated . The employee will need to sign the " last chance <br /> agreement" twice , once at the time of entry into treatment and once upon his or her return to work. <br /> When the employee returns to work , any treatment recommendations will be attached to the <br /> agreement. <br /> Violation of this policy will result in disciplinary action , including , but not limited to a warning , written <br /> reprimand , suspension , dismissal , expulsion , and/or mandatory participation and successful <br /> completion of a drug and alcohol abuse assistance or rehabilitation program approved by an <br /> appropriate health or law enforcement agency. <br /> Treatment <br /> The Company's health plan has some benefits for the treatment of drug and/or alcohol abuse for <br /> covered employees and their covered dependents . Detailed information may be obtained by calling <br /> the health plan or through reading the plan booklet(s) available in the Office of Human Resources . <br /> Disciplinary Action <br /> The Company's officials must initiate disciplinary acting upon first finding of illegal drug use and/or <br /> prohibited alcohol conduct. The severity of the disciplinary action is dependent on the circumstances <br /> of each case and is consistent with the DOT regulations . Company officials must initiate action to <br /> remove the employees from service when employees: <br /> 1 . Refuse to obtain counseling or rehabilitation through the "Last Chance Agreement" ; <br /> 2.
