The boxes storing hazardous waste were labeled ORM-D. All containers of hazardous <br /> waste must be labeled as required in Title 22 Hazardous Waste Regulation. Correct <br /> Immediately. <br /> Spent absorbent has been placed in plastic bags and then boxed for shipment. <br /> Additionally, boxes of non-functional aerosol cans containing hazardous waste were <br /> observed open. Hazardous waste must be stored in a closed container compatible with the <br /> waste being stored within. The container must be kept closed all times except when <br /> adding or removing contents. Begin this practice immediately. <br /> The hazardous waste items mentioned above were shipped from the Claims Department <br /> and transported on a Bill of Lading. Transportation occurred via a Wal-Mart truck, which <br /> was not registered as a hazardous waste hauler. The disposition of the hazardous waste <br /> occurred at an unauthorized point, which was the Wal-Mart return center in Las Vegas, <br /> Nevada. All hazardous waste must be transported under a manifest and by a licensed <br /> hazardous waste hauler. Begin this practice immediately. <br /> Immediately being accumulating, storing and transporting hazardous waste as required <br /> in California Code of Regulations Title 22 Hazardous Waste Regulations. <br /> • #49 CCR Section 66265.16 Personnel Training Records incomplete <br /> The personnel handling the hazardous waste were not properly trained. Personnel were <br /> unaware of the proper storage and transportation requirements for handling hazardous <br /> waste. At the time of inspection, personnel did know what a manifest was and how it is <br /> suppose to be used. Immediately train all personnel who handle hazardous waste. <br /> Submit a Certification of Return to Compliance with a letter detailing how each violation <br /> has been corrected. Include a copy of personnel training records for hazardous waste <br /> handling, hazardous waste manifests for the shipment of waste currently onsite, and a <br /> description of how the Claims Department accumulates, stores, and transports hazardous <br /> waste. Submit to this office by November 30, 2005. <br /> Inspector Received By <br />
