Refer to Appendix F—Discharge Notification Form <br /> The form will assist you in gathering the necessary information to be <br /> reported to the emergency authorities. <br /> 1. If there is a fire or injury, on duty supervisor/manager or his/her designee <br /> must immediately call 911. <br /> 2. If possible, stop the flow of fuel/oil by shutting a valve, turning off a pump, up- <br /> righting a container, or temporarily plugging a rupture. Only do so as <br /> conditions safely permit. If it is unsafe to stop the flow, call 911. <br /> 3. Isolate and contain the spilled material by using absorbent, spill booms, spill <br /> kits, or creating an earthen berm with a shovel or other available equipment <br /> (beware of fire danger). <br /> 4. Estimate the amount of spilled material. <br /> 5. Make all of the appropriate notifications listed in the Emergency Contact <br /> section. <br /> 6. Use available cleanup equipment and/or the spill contractor to clean up the <br /> spilled material and contaminated soil/surfaces. <br /> Document all spill response and cleanup efforts, including notification calls. <br /> Refer to Appendix F—Discharge Notification Form <br /> Document the following information for each agency contacted: <br /> • Agency notified; <br /> • Time agency notified; <br /> • Person notified; <br /> • Content of message given; and <br /> • Follow-up requirements. <br /> 21 Page <br />
