MURO <br />11/01/2021 Called Carlos Sanchez at American Integrated Services, Inc. (AIS) at MURO <br />707-210-5529. Phone call did not go through. Called AIS Office phone number <br />707-427-2234 and the receptionist hung up on me. Called Jim Clark at <br />870-715-6207. Reprinted an updated log and 5105 report. <br />MURO <br />01/11/2022 Sent an email to Rachel Brosky-Kish <rbrosky@ertsonline.com> reporting agency <br />for FedEx and debra.gehm@fedex.com asking who would be a good person to <br />ask concerning waste disposal records for this complaint. What was classified as <br />MANZO <br />RCRA hazardous waste during clean up was taken to 801 Diamond facility <br />(FA0002969) without a hazardous waste manifest. Sent an email to C. Muro (cc <br />to G. Backus EHD), to make a note under HW PR for that facility to be followed <br />up on during the routine hazardous waste inspection. Received a voice message <br />from Kelby 513-390-2684 with FedEx. Called Kelby back let him know container <br />number and spill details to see if FedEx had the waste picked up and disposed of <br />or if the waste was handled by the BNSF. Clean up was completed. <br />01/11/2022 Cross-Referenced with facility at 801 Diamond St [PR0220061] since American MURO <br />Integrated Services performed cleanup and generated two 55-gallon drums of <br />RCRA Hazardous Waste and transported to BNSF faciility located at 801 <br />Diamond St without a manifest. <br />Reprinted reports 5104, 5105, and narrative log.
