Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: CO3DKMIRINV Site Location: 6540 S AUSTIN RD <br />Activity Date <br />Activity Summary <br />Recorded by <br />07/21/2020 12:00 AM Continued from last-night. I was still at the leaker pit area. Consulted MURO <br />Elena Manzo regarding AIS intention of transporting the hazardous waste on a <br />bill of lading. Elena Manzo provided the California Health and Safety Code - HSC <br />25110.10 which defines the consolidation site as having requirements and <br />conditions that must be met before the facility can be considered a consolidation <br />site. I was advised to let Carlos Sanchez (AIS) know of this code, but if he still <br />chooses to transport the two 55-gallon drums of generated waste, not to stop him. <br />I made recommendation to Carlos Sanchez, but he stated he will be transporting <br />the waste to the 801 Diamond St. BNSF staging area. <br />Carlos Sanchez escorted me to lot 174 so that I can take a look and photos of the <br />spill area after cleanup. I did not detect any smell of paint lacquer. The spill area <br />was dry and cleaned up adequately. <br />I began to generate complaint while the bill of lading was written out. <br />I took photo of the bill of lading. <br />Left area and again had issues getting out, since the crossing gate was not lifting. <br />Called John Brainard for assistance, and he said to wait there, it will eventually be <br />lifted. <br />1:06 AM END <br />07/21/2020 Updated Elena on the complaint details. MURO <br />05/27/2021 Follow-up email to Jim Clark (FEDEX) at 'Jim.Clark@fedex.com' and Juanita MURO <br />Gibbany (FEDEX) juanitta.gibbany@fedex.com requesting requested items. <br />05/27/2021 Review complaint. MURO <br />06/01/2021 Received response from Juanitta Gibbany. She advised I forward this complaint <br />to Steven Chaney at steven.chaney@fedex.com.
