Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: CO8UZCGNC Site Location: S 99 FRONTAGE RD & OLIVE AVE <br />Activity Summary <br />Activity Date Recorded by <br />12/07/2021 I received an email from Anthony Alejo informing me that the drums of diesel was VANG-LEE <br />removed from the site location but he did not provide any update regarding the <br />excavated soil. I made contact with Derek Smith at 916-347-8724 who stated that <br />the roll-off bin of soil is still at the site location and they will work on disposing of of <br />the waste this week and provide analytical results to the EHD. <br />VANG-LEE 01/06/2022 I attempted to contact Derek Smith of Patriot at 916-347-8724 but made contact <br />with Matt Musolf instead. Per Matt, Derek has resigned. Matt requested that I text <br />him at 916-347-8717 with a summary of the complaint and he will follow-up with <br />me once he gets back into th office. <br />I received a call back from Matt Musolf and Ambrose Kong (project manager) of <br />Patriot Environmental. Per Ambrose, they received the analytical results but the <br />disposal facility would not accept the waste because the results did not include the <br />8260 method. They stated they will try to find another disposal facility that will <br />accept the waste. If they cannot find a disposal facility that will accept the waste, <br />they will run the 8260 test. <br />01/11/2022 I received the analytical results via email from Matt Musolf <br /><mmusolf@patriotenvironmental.com> on 01/06/2021. I also received an email <br />from Ambrose Kong <Ambrose.Kong@patriotenvironmental.com> the same day <br />stating, "The current plan is to go get another sample, run another analytical using <br />Method 8260, and then attach it with the current one to be resubmitted for profiling <br />with a WM facility." <br />02/25/2022 I contacted Ambrose Kong of Patriot Environmental regarding the status of the <br />waste analytical. Per Ambrose, they did not conduct further analysis of the waste <br />as planned. Ambrose stated that Kirby Canyon Landfill accepted the waste <br />without requiring the addition 8260 test. Ambrose stated that the waste was <br />picked-up on 01/13/2022 and he will provide a copy of the manifest and waste <br />profile that was provided to the landfill. <br />02/28/2022 Received a copy of the waste profile that was submitted to Kirby Canyon Landfill <br />and a copy of the non-hazardous waste manifest NH11-21-00483 for 15 yards of <br />"soil contaminated w/ diesel". I have not received the final manifest for the two <br />diesel drums that was removed from the truck's saddle tanks on 09/07/2021. I <br />looked up the manifest on the Department of Toxic Substances (DTSC) tracking <br />system and printed the final copy for manifest 019704500JJK. <br />Reviewed analytical results for the excavated diesel contaminated soil. The <br />sample was tested for the following metals: antimony, arsenic, barium, beryllium, <br />cadmium, chromium, cobalt, copper, lead, molybdenum, nickel, selenium, silver, <br />thallium, vanadium, and zinc. The sample was also tested for VOCs. C12-C22 <br />hydrocarbon levels were at 421 mg/kg. All results were under the threshold and <br />the waste was disposed of as nonhazardous. <br />Generated the complaint investigation form (5104), emergency response records, <br />site map, complaint investigation summary (5105), and the daily time and activity <br />records (Log2).
