Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: CO8UZCGNC Site Location: S 99 FRONTAGE RD & OLIVE AVE <br />Activity Summary <br />Activity Date <br />10/04/2021 Contacted Derek Smith of Patriot Environmental at 916-347-8724 at 7:33am to get <br />an update of the scheduled clean-up today. Derek stated that clean-up has not <br />started. I advised Derek to notify me approximately 30 minutes before completing <br />excavation so that I could verify the clean-up before they backfill. <br />After receiving a call from Derek, I met Derek at the site location. I assessed the <br />area and informed Derek of the areas of concern. Derek stated he was unable to <br />continue excavating without damaging the road. I contacted lead Elena Manzo to <br />discuss the clean-up. I contacted Brian Closs of San Joaquin County Public <br />Works Road Maintenance at 209-468-3074. Brian stated someone will be on site <br />to assess the road. John Moore of road maintenance arrived at the site. Per <br />John, additional excavation can be performed if needed. Additional excavation <br />was performed on the shoulder of the road. Clean-up is complete and site was <br />approved to backfill. The excavated soil was placed into a 20 yard roll-off bin. Per <br />Derek, the bin will be left at the site and disposed of once they receive the <br />analytical results. Per Derek, the two 55 gallon drums of diesel that was <br />generated on 09/07/2021 has been transported to Patriot's location in Richmond <br />and will be transported to Crosby and Overton in Long Beach, CA. A photo of <br />manifest 019704500JJK was texted to me during the clean-up. <br />I returned to the office and completed photo log of clean-up. Emailed a copy of <br />the spill report and responsible party contact information to Brian Closs per his <br />request. <br />10/14/2021 I received a call from Sergeant Barbara Butler from 209-938-4800 inquiring about <br />the status of the clean-up. I informed Sergeant Butler that the clean-up was <br />completed on 10/04/2021 and the EHD still needs to follow-up on the waste <br />disposal. Sergeant Butler stated she would send me a copy of her report when <br />she was done with it. <br />Recorded by <br />VANG-LEE <br />VANG-LEE <br />10/20/2021 Reviewed the Hazardous Materials Incident Report I received from Sergeant VANG-LEE <br />Barbara Butler on 10/14/2021. <br />10/26/2021 Left a voicemail message for Derek Smith of Patriot at 916-347-8724 requesting VANG-LEE <br />for an update regarding the waste determination and disposal for the excavated <br />soil. Derek returned my call and stated that the roll-off bin was still at the site <br />location and he could not confirm if they have received the waste determination <br />back. Derek states he will update me once he has more information. <br />11/05/2021 Left a message for Derek Smith at 916-347-8724 requesting for an update VANG-LEE <br />regarding the waste that was left at the site location. <br />11/09/2021 Emailed Anthony Alejo <anthony_alejo@ryder.com> requesting for an update. I VANG-LEE <br />informed Anthony I have not heard back from Patriot Environmental and still need <br />the analytical results, disposal records, and clarification whether the excavated soil <br />is still at the site location. <br />11/15/2021 I contacted Derek Smith of Patriot regarding the analytical results and disposal VANG-LEE <br />records. Derek stated he will provide the requested documents to me this week. <br />12/06/2021 Emailed Anthony Alejo <Anthony_Alejo@ryder.conn> and Derek Smith VANG-LEE <br /><dsmith@patriotenvironmental.com> for an update regarding the analytical results <br />and disposal records for the excavated soil. <br />5105 rpt <br /> Page 3 of 4
