Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00053721 Site Location: 2325 W LOUISE AVE <br />Activity Summary <br />Activity Date <br />Recorded by <br />05/07/2021 Received a call from Kelly stating PSC was done with the clean-up. Left EHD for FLORIDO <br />site. <br />Arrived on-site and met with Kelly, and Brandon of PSC, and observed the <br />excavation around the base of the emergency generator concrete slab. The <br />excavation was approximately 3 feet deep, 10 feet long and 14 inches wide on the <br />north side of the generator, and 5 feet long and 10 inches wide on the east side of <br />the generator. The clean-up is complete. PSC generated two 1-cubic yard boxes <br />of hazardous waste solids. Took photos. <br />Kelly escorted me to the facility's hazardous waste storage area, where I observed <br />the 55-gallon drum of hazardous waste solids generated from the cleanup on <br />4-24-21. Took photos. <br />Kelly stated she has not yet heard back from Booster fueling contractor regarding <br />where they took and how they managed the absorbent pads and powder they <br />used to initially clean the spill on the date of the incident. Left site for EHD. <br />Generated photo log and emailed Kelly confirming the clean-up is complete and I <br />still need the information regarding the waste generated by Booster. <br />06/04/2021 Reviewed an email received on 5/12/21 from Kelly notifying me that the absorbent FLORIDO <br />initially used to clean up the spill was taken to 2278 Pike Ct. Concord, CA 94520, <br />and will ultimately be picked up by Clean Harbors. It was approximately 8-10 <br />gallons of solid materials. Kelly also stated the 2 cubic yard boxes and 1X55gal <br />drum of soil that was excavated would be picked 5/13/21 by IDR Environmental. <br />Looked up 2278 Pike Ct. Concord on CERS and found it is the address to Pacific <br />States Petroleum. <br />Emailed Kelly requesting her to confirm if Booster (fueling contractor) transported <br />the 8-10 gallons of used absorbent to Pacific States Petroleum, 2278 Pike Ct, <br />Concord, CA. <br />Generated photo log of initial spill. <br />06/04/2021 Reviewed and organized complaint documentation. FLORIDO <br />01/18/2022 Called and notified Kelly Durrer, responsible party, to pay complaint fees by 2/1/22 FLORIDO <br />otherwise fees will go to collections. Emailed Kelly a copy of the invoice. <br />01/31/2022 On 6/7/21 Kelly responded confirming Booster (fueling contractor) transported the FLORIDO <br />8-10 gallons of used absorbent to Pacific States Petroleum, 2278 Pike Ct, <br />Concord, CA, which would then be picked up by Clean Harbors for proper <br />disposal. <br />Reviewed narrative, printed remaining complaint documentation, including final <br />hazardous waste manifest for the disposal of the waste generated from the <br />clean-up.
