Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: C00053721 Site Location: 2325 W LOUISE AVE <br />Activity Summary <br />Activity Date <br />Recorded by <br />04/24/2021 Time Spent: 9:36 AM-12:36 PM <br />ALANIZ <br />9:39 AM Received a call from Kelly Durr, 209-305-0171. Medline hired PSC, an <br />environmental contractor, to clean up the diesel spill. Kelly said that PSC had <br />almost completed the cleanup. Left to the site and met with Kelly and PSC <br />contractors. Soil with diesel staining was observed at the base of the cleanup <br />area at the north end of the generator. The contractors had dug a 6 inch deep, 8 <br />inch by 3 foot hole. Soil with diesel staining was also observed at the base of the <br />cleanup area at the east end of the generator. The contractors had dug a 4 inch <br />deep, 4 inch by 2 foot hole. <br />PSC could not dig any further until their survey by USA dig had been completed. I <br />explained that the spill was not clean by county standards until all of the diesel had <br />been dug up. Discussed rain events with Kelly and advised her to watch the area <br />and upcoming storm in order to prevent further diesel contamination in the soil by <br />the rain water. Approximately 3/, of one 55 gallon drum of diesel debris was <br />generated. The facility is going to handle the waste as hazardous waste and store <br />it in their hazardous waste storage area. <br />Completed the photo log and narrative. <br />04/26/2021 Generated Prop 65 FLORIDO <br />04/27/2021 Emailed Kelly Durrer reminding her to let me know when the spill clean-up will be FLORIDO <br />completed so that an inspector can verify the clean up is complete prior to backfill. <br />Received an email from Kelly stating she will let me know when the clean-up is <br />complete before backfilling, and that the clean up was started on 4/24/2021 but <br />stopped due to a USA Dig was needed and scheduled for 4/28/2021. Kelly stated <br />once USA Dig approval is received the clean-up will continue. Kelly stated the spill <br />area was tarped on Sunday to ensure no ensure the forecasted rain did not come <br />into contact with the spill area. <br />Emailed Kelly reminding her to also also let me know how much used absorbent <br />and pads were generated on the date the spill occurred, by who, and where that <br />waste was transported to. <br />05/03/2021 Reviewed an email received from Kelly on 4/30/2021, in which she states she <br />received approval from US Dig to continue digging, she reached out to have the <br />clean-up contractor return next week, and she does not know what day they will be <br />onsite, but she will let me know once she receives confirmation. Emailed Kelly <br />asking her if she was able to find out the information regarding the clean-up waste <br />generated by the fueling contractor on the date of the incident. <br />05/04/2021 Received an email from Kelly stating a clean up contractor will be on-site <br />tomorrow at 8am to continue the clean-up and she will let me know before they <br />backfill the area. Kelly also stated Booster was the fueling contractor onsite on the <br />date of the incident and who generated the initial cleanup waste. She provided a <br />statement from Booster which states the 2 gallon release was cleaned with <br />absorbent pads and dry sweep, and all cleanup materials were disposed in <br />compliance with US EPA guidelines using Clean Harbors Receptacle and service. <br />05/05/2021 Received an email from Kelly stating she was on-site to meet the clean up crew <br />today at 8am, but they informed her they wouldn't be at the site until tomorrow at <br />8am. I emailed Kelly requesting the name of the clean up contractor and to <br />provide the address or phone number for the location where Booster took the <br />clean up waste generated on the date of the incident. <br />FLORIDO <br />FLORIDO <br />FLORIDO <br />5105. rpt <br /> Page 2 of 4
