Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: C00054190 Site Location: 14780 S HARLAN RD <br />Activity Summary <br />Activity Date <br />08/24/2021 <br />Recorded by <br />Arrived onsite, met with the truck driver Allen Edwards 262-939-9541 who was MANZO <br />waiting for the truck tow onsite. The black oily diesel stain area was under the <br />truck parked behind the Walgreens store, some dry absorbent was applied to the <br />stained area. Dark staining extended northward toward the back curb, measuring <br />approximately 15x30 feet. No storm drains were observed in the area of the spill. <br />All Environmental Services Inc (All Environmental) clean up crew arrived at 5:45 <br />pm. 5:46 pm called Yves Thomas with CPC logistics, provided an update, Mr. <br />Thomas indicated that my inspection report should be emailed to: <br />y.thomas@cpclogistics.com and kevin.anderson@walgreens.com. All <br />Environmental applied additional absorbent to the diesel stain area and removed <br />residual diesel fuel from the broken down truck. When I asked what will happen to <br />the waste, All Environmental clean up crew representative (Robert Hinson) <br />indicated they will haul it away, but he was not sure if a hazardous waste manifest <br />will be generated and how the drums will be labeled (indicating he will check with <br />office at a later time for directions). Since it was unclear what will happen with the <br />waste I remained onsite. 7:08 pm, received a call from Brandon Espinoza with All <br />Environmental, who indicated that waste should be labeled as hazardous and sent <br />off-site on a hazardous waste manifest (he is working on obtaining a temporary ID <br />number). Brandon asked if EHD will require to have the area pressure washed, <br />and I indicated that if absorbent clean up is deemed adequate, no pressure <br />washing is required, however, I also let him know that All Environmental should <br />check with the property owner to see if any additional clean up will be required. <br />7:15 pm, called Mr. Thomas, provided an update, let him know that I ended up <br />remaining onsite for the duration of the clean up since clean up crew was unclear <br />what will happen with the waste and although Brandon indicated that waste will be <br />labeled I was not able to confirm it with the onsite crew (per Javier, with onsite <br />clean up crew they will reach out to their office later to see what needed to be <br />done for waste disposal). Also let Mr. Thomas, know about my conversation with <br />Brandon regarding pressure-washing and if they decide to pressure wash to let <br />EHD know ahead of time and make provisions for containerizing and disposal of <br />all generated rinsate. 7:20 pm, tow truck arrived to move the truck so the clean up <br />under the truck could proceed. All Environmental pumped two 55-gallon drums of <br />diesel from the damaged truck, swept and containerized all the absorbent into one <br />55-gallon drum, all three drums were labeled with hazardous waste labels and <br />transported offsite by All Environmental under a hazardous waste manifest <br />003623316 GBF. Generated inspection OIR and emailed to <br />y.thomas@cpclogistics.com; kevin.anderson@walgreens.com; cc to Elianna <br />(eflorido@sjgov.org), documenting spill clean up and requesting final disposal <br />records. Updated narrative. Clean up is complete. 5:00 pm- 8:48 pm <br />08/25/2021 Discussed spill clean-up with Elena Manzo, Ld Sr REHS. Documented narrative. FLORIDO <br />08/25/2021 Finalized complaint narrative, downloaded inspection pictures, created picture file, MANZO <br />provided pictures to E Florido. <br />10/11/2021 Reviewed narrative. FLORIDO <br />10/11/2021 Called and left a voice message for Yves Thomas. Emailed Yves Thomas and FLORIDO <br />Kevin Anderson requesting they provide a billing address and the final hazardous <br />waste manifest for the three 55 gallon drums of spill clean-up waste. Received the <br />responsible party billing information from Yves Thomas. Updated the billing <br />information in Envision. <br />01/31/2022 Reviewed manifest for waste generated from clean-up, recieved 10/13/21 from FLORIDO <br />Yves Thomas.
