Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COAOXB82Q Site Location: 1000 ROTH RD <br />Activity Summary <br />Activity Date Recorded by <br />11/03/2021 Called Derek who stated he is almost at the facility to begin clean-up of the FLORIDO <br />container. Requested Derek let me know what material spilled. Called and spoke <br />to Jeryll Womack, UPS Regulated Goods Coordinator (614-332-6548), notifying <br />her that I will be emailing her a Notice To Abate and she will need to provide the <br />final waste disposal records. Generated Notice To Abate and emailed it to Jeryll <br />(jlwomack@ups.com). Received photos from Derek showing that a 5-gallon <br />container of Synergy III HS Process Extender PR-1 - flammable printing ink <br />related material, was punctured and spilled. Derek stated that the container that <br />spilled still had about 2-3 gallons of liquid material that he will add absorbent to in <br />order to solidfy it for waste disposal. Notified Derek that I would be heading to the <br />site shortly to verify clean-up. Arrived on-site and verified the clean-up was <br />complete. One 55-gallon drum of waste was generated. Derek stated the waste <br />would be disposed of as non-RCRA hazardous waste. After a discussion with <br />Elena Manzo and Garrett Backus, Ld Sr REHSs, I notified Derek that because the <br />spilled material left over in the 5-gallon container was flammable liquid before he <br />solidified it, a hazardous waste determination would have to be made on the liquid <br />waste, before the absorbent was added, which means that the 55-gallon drum of <br />waste is likely RCRA hazardous waste. Derek consulted with his manager. Derek <br />showed me the hazardous waste manifest 019704495JJK which shows one drum <br />of D001 RCRA solids containing flammable liquids. I took photos of the clean up, <br />the waste, and the hazardous waste manifest. Left site. <br />11/04/2021 Generated Prop 65 FLORIDO <br />11/05/2021 Reviewed and revised narrative. FLORIDO <br />11/05/2021 Completed Emergency Response Record form FLORIDO <br />11/10/2021 Received an email from Jeryll Womack notifying me that I made a typo on the FLORIDO <br />responsible party mailing address. I corrected the mailing address in EC and on <br />the Prop 65. Prop 65 was amended and re-mailed. <br />12/01/2021 Received a generator copy of the hazardous waste manifest from Jeryll Womack. FLORIDO <br />Reviewed narrative. Emailed Jeryll requesting she submit the designated facility <br />signed copy of the manifest once she receives it. <br />02/01/2022 Generated photo log. FLORIDO <br />02/01/2022 Printed and organized complaint documentation. Submitted complaint to Elena FLORIDO <br />Manzo, Ld Sr REHS, for abatement. <br />51carpt <br />Page 2 of 2
