Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COARKVI3W Site Location: 4929 COLLIER RD <br />Activity Summary <br />Activity Date Recorded by <br />11/03/2021 Received a text message from Alex Steele asking where! found the oil in the FLORIDO <br />ditch. I told him next to the first driveway west of the transformer pole, past the <br />boom. Alex stated he's going to the site and will be there in 1 hour. I told him I <br />would meet him on site. Prepared for inspection and left EHD. Arrived on site and <br />met with Alex. The boom observed on 11/2/2021 was still in the ditch. Showed <br />Alex where I observed the oil gobules. Oil was still in the water in that location. Oil <br />was also observed about 200 feet west of the impacted transformer pole. Alex <br />stated Dave Larsen is on his way from Stockton with oil absorbing pads. Took <br />photos of the oil on the water in the ditch. Dave arrived on-site. Alex and Dave <br />absorbed as much oil as they could with the absorbent pads. Alex moved the <br />boom downstream from where the oil was observed, in order to absorb any <br />remaining oil. Alex and Dave placed the used pads into a 20 gallon drum, which <br />Dave stated would be taken to the PG&E service center at 4040 West Ln in <br />Stockton. Dave stated they will leave the boom and three pads in the ditch until <br />tomorrow to absorb any remaining oil. Dave stated he would removed the <br />remaining boom and pads tomorrow and send me photo of the area showing the <br />waterway is clean. Took photos of the remaining boom and pads. Left site for <br />EHD. <br />11/04/2021 Requested Alex Steele provide a photo of the ditch once the remaining boom and FLORIDO <br />absorbent pads are removed, in order to verify final clean-up. Alex agreed. <br />Generated Prop 65. <br />11/05/2021 Reviewed and revised narrative. FLORIDO <br />11/05/2021 Completed Emergency Response Record form FLORIDO <br />02/02/2022 **Reviewed photos received from Dave Larsen on 11/12/21 showing the FLORIDO <br />absorbent pads and boom were picked up, placed in the 20-gallon drum to be <br />transported to the PG&E facility at 4040 West Ln in Stockton. Clean-up is <br />complete. Emailed Alex Steele requesting he submit the PCB analytical. <br />Generated photo log. <br />02/02/2022 Printed and organized complaint documentation. FLORIDO <br />02/03/2022 Reviewed PCB analytical received on 2/3/22. The oil sample was found to be FLORIDO <br />non-detect for PCBs. See PR0513829 for waste disposal records. Complaint <br />ready for abatement. <br />02/07/2022 Printed remaining complaint paperwork and submitted complaint to Elena Manzo, FLORIDO <br />Ld Sr REHS, for abatement. <br />5105 rpt Page 2 of 2
