Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COE8QXMIA Site Location: 1495 E PESCADERO AVE <br />Activity Summary <br />Activity Date Recorded by <br />12/17/2021 5 pm- Reviewed spill report information for Cal OES Control #:21-7242 received MANZO <br />today at 4:03 pm. Called E. Florido (EHD) who is on C today, no answer. Called M. <br />Nissim (program coordinator) to discuss daytime spill response. Received spill <br />information from Elianna Florido (EHD)- via phone and email; EHD has been <br />receiving conflicting information concerning the spill clean up from the clean up <br />company; facility is not returning phone calls and as such the conflicting <br />information couldn't be verified over the phone- as such a site visit to verify clean <br />up was required. Reviewed facility CERS information, reviewed complaint email <br />with facility contacts received from E. Florido. <br />Arrived onsite, met with a female office worker (no name provided) to inquire <br />about the spill clean up, she indicated that she was not sure and called Enrique <br />Aguilar Operations Manager, (209) 832-7173, to verify clean up details. Enrique <br />reported that the muriatic acid spill occurred from a pallet stored on the concrete <br />facility floor near the dock door 166 and that no shipping container was impacted <br />by the spilled material. Inspected the hazardous waste storage area and observed <br />two blue poly containers (55-gallon and 35-gallons) labeled as hazardous waste. <br />The 55-gallon container label indicated "muriatic acid 26-1 gallon" and the <br />35-gallon container "muriatic acid cardboard", with the accumulation start date of <br />12/17/2021. Inspected the area near the dock door 166, observed a pallet of <br />muriatic acid, no leaking or liquid was noted around the pallet. Observed several <br />drops of liquid approximately 5 feet away from the pallet, pH of 8.5-9, facility <br />personnel applied absorbent to the liquid drops and will be disposing the <br />generated wastes as hazardous. Clean up was deemed adequate. Left the site, <br />generated an official inspection report documenting the site visit observations. <br />Emailed OIR to Byerley, Ruben <Ruben.Byerley@YRCFreight.com>; <br />enrique.aguilar@yrcfreight.com; Elianna (eflorido@sjgov.org). Documented <br />complaint.(5pm-7:48pm) <br />12/20/2021 Downloaded inspection pictures, generated picture log, ennailed picture log to E. MANZO <br />Florido (EHD). <br />02/03/2022 Reviewed narrative. Generated Prop 65 and Emergency Response Record form. FLORIDO <br />See PR0527431 for final disposal records. Complaint ready for abatement. <br />02/07/2022 Printed and organized complaint documentation.
