Let her know I'm currently on the way back to the office, she left
the person's information at my desk for when get back.
Checked the address in Envision Connect, this is a regulated business on
inventory. Called back phone number and spoke to Matt Bernal, who said at the
time when he called it was 1-gallon that had spilled, but since it is actively leaking,
it is now at about approximately 2-gallons. Matt stated that the spilled substance
was a premium hydrogen peroxide which is a corrosive and oxidizing liquid. He
called ERTS (Emergency Response and Training Solutions as part of their spill
response and they advised him to leave the spill as is and isolate the area. The
clean up company is expected to arrive to their facility clean the spill at around
8:30pm. He said the odor is very strong so he conned off and isolated the area to
keep his staff safe. He did not want to get close to the spill due to not having have
the correct PPE but was able to see that the leaking container is a 5-gallon plastic
container. The container was damaged in the trailer and is continuously leaking
from the trailer to the concrete.
Called Matt back to request a copy of the SDS. He said he doesn't have an SDS
for the material but he does have the bill of lading, he will email me a copy of it.
The bill of lading will have information on the chemical.
Received a spill report for the spill, reporting party is ERTS.
12/10/2021 Called lead senior to update on spill. Forwarded bill of lading received from Matt to PRESTO
lead senior to as I couldn't look at the email due to heading to a different spill.
5105. 'pt Page 1 of 2
