Complaint Investigation Summary Report* 5105 <br />COMPLAINT ID: COKQAKOX2 Site Location: 5475 S AIRPORT WAY <br />Activity Summary <br />Activity Date <br />12/10/2021 I contacted lead Elena Manzo to discuss the complaint. I left a message for <br />Makayla Martin at 904-569-0071. I received a call back from TJ Shay of ERTS <br />(904-727-0776) providing me with John Soria's (of Ancon) contact number. I <br />contacted John regarding clean-up. Generated a complaint record in Envision. <br />Met with John Soria of Ancon at the site location. According to John, 5 gallons of <br />hydrogen peroxide was released from a 5 gallon bucket and impacted a wooden <br />pallet, the trailer floor, and concrete pad (parking area). Per the spill report, the <br />release was caused by improper blocking and bracing of the freight. Per John, <br />absorbent was applied to the impacted trailer floor and concrete pad and then <br />neutralized with sodium bicarbonate. Three 55 gallon poly drums of hazardous <br />waste was generated. One 55 gallon poly drum was for the impacted wooden <br />pallet. The second 55 gallon poly drum was for the used absorbent. The third 55 <br />gallon poly drum was for the rags. The waste will be left on site and disposed as <br />hazardous waste at a later time. Clean-up is complete. <br />Generated and emailed an inspection report to Jeff Sexten <br /><Jeff.Sexten©xpo.com>. <br />(8pm - 10:30pm) <br />12/13/2021 Completed photo log. <br />12/14/2021 Mailed official complaint inspection report dated 12/10/21 by regular mail to: <br />CF PROPERTIES INC, <br />PO BOX 3745, <br />PORTLAND, OR 97208. <br />12/14/2021 Entered complaint record information for property owner and GIS. Drafted Prop65 <br />and placed into mailing bin. <br />02/07/2022 Printed documents for abatement packet.
