Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COWLXUNSH Site Location: JUDAH WAY & HOPKINS RD <br />Activity Summary <br />Activity Date Recorded by <br />01/13/2022 Reviewed email from Carol Presto EHD for clean up for FedEx Ground in Tracy ERNEST <br />Ca. She indicated the clean-up for this site has been completed, approx 25 gallons <br />of waste was generated. <br />01/13/2022 Emailed update of clean up to Elena Manzo, Ld Sr REHS, Melissa Nissim, CUPA PRESTO <br />Program Coordinator, Michelle Henry, CUPA Program Coordinator and Elianna <br />Florido, Sr REHS. <br />PRESTO 01/13/2022 On site for clean up verification. Ashkan "Ash" Moghaddam, clean up supervisor <br />with Patriot Environmental and Steffany Aguilar, project geologist with Farallon <br />Consulting-representative of property owner Prologis were on site. Two areas <br />were excavated that had motor oil. Excavation sizes were one circular area with <br />an approximate diameter of 1.5ft and a bean shape approximately 3.5ft by 2.8 ft. <br />Both had a depth of approximately 6 inches. Waste generated was placed in a <br />55-gallon drum. Approximately 35-gallons of waste were generated. The drum will <br />be hauled away by Patriot Environmental Services. The area was not backfilled as <br />that was not part of Patriot's job duty for the project. Per Ash, we can call him to <br />schedule a backfill if needed. Called CUPA Program Coordinator to verify if we <br />need the area backfilled, they are shallow holes in an undeveloped plot of land. No <br />backfill needed for our part. Informed Steffany that if Prologis would like the area <br />backfilled, they can contact Patriot to schedule a backfill. Ash stated the other <br />areas of staining were no longer at the site when he did a site visit last week. Per <br />Ash, the staining must have dissipated from the rain and he suspects the staining <br />was coolant rather than oil or fuel. He checked the area for any oily feelings and <br />odors but none were observed. Walked around the gravel/soil area, no other <br />staining was observed. FedEx representative was not on site. From Steffany and <br />Ash's knowledge a FedEx representative was going to be on site. Called Debra <br />Gehm, FedEx environmental contact, to see if she was aware of a FedEx <br />representative coming to the clean up. Per Debra, no FedEx reps were coming. <br />Informed Steffany and Ash. Ash said he will send me a copy of the manifest after <br />he receives the temporary EPA ID number from his office. Left the site for EHD. <br />Received photos from Ash of manifest and hazardous waste drum. Saved photos <br />into P drive. Created photo log for photos taken and photos received from Ash. <br />Made a copy of Steffany and Ash's business cards for the packet. Placed all in <br />area inspector's inbox. <br />01/18/2022 Reviewed photo log of clean-up, received from Carol Presto, REHS. Left voice <br />messages for and sent emails to Debra Gehm of Fedex and Steffany Aguilar of <br />Farallon Consulting (Prologis representative), asking if trucks will continue to be <br />parked on the Prologis property, and if yes, will a land use approval be obtained. <br />Received email from Gavin Polite Fisco, Prologis Global Environmental & <br />Engineering Services Manager, stating Fedex was not given permission to park on <br />this property, and Prologis has provided notice that they are not allowed access to <br />this property in the future and they have acknowledged receipt. Future parking <br />violations should be forwarded to Gavin directly(gfisco@prologis.com; <br />415-733-9410). <br />FLORIDO <br />01/20/2022 Emailed Debra requesting she provide the final hazardous waste manifest, signed FLORIDO <br />by designated facility, in order to abate complaint. Received the generator copy of <br />the manifest (021913641JJK) from Debra, who stated she will provide the final <br />copy of the manifest once she receives it. 220 pounds of non-RCRA hazardous <br />waste solid (oily dirt & debris) was disposed. <br />01/24/2022 Reviewed certified mail return receipt received 1/7/2022, confirming a person with FLORIDO <br />signature H Burnside received the 11/8/21 Notice To Abate on 12/20/21 on behalf <br />of Prologis LP. <br />51 05. rpt Page 4 of 5
