Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COWLXUNSH Site Location: JUDAH WAY & HOPKINS RD <br />Activity Summary <br />12/21/2021 <br />01/07/2022 <br />01/11/2022 <br />01/11/2022 <br />Provided Vicky Vang-Lee, REHS, the complaint information, since she may be <br />called to verify the clean-up during after-hours. <br />Discussed possible after-hours clean-up with Elianna Florido. Reviewed <br />narratives, photos, and site map. <br />Received an email from Debra on 11/23/21 stating Fed Ex and Prologics, property <br />owner, are going to finalize a property access agreement before Fedex performs <br />the clean-up and she will provide me 48 hours notice before the clean-up. <br />Emailed Debra today asking her for a status update. <br />Time spent determining if Notice To Abate to Prologics, property owner, and Prop <br />65 were sent out. Determined they were not. Notice To Abate to Prologics and <br />Prop 65 sent out today. Discussed complaint with Muniappa Naidu, EHD Asst <br />Director. <br />Called Debra Gehm, FedEx Environmental Compliance Specialist (412-216-3701), <br />left a voice message asking for the status of access agreement and the site clean <br />up schedule. Provided my contact information. <br />Received green certified mail receipt in the mail. Date of delivery was 12/20/21 <br />and was signed by H BURNSIDE. Certified mail #7021 0350 0000 8150 0813. <br />Scanned to inspector Florido's folder. <br />Sent Fred Earnest the photos of the spill area, since he will be verifying the <br />clean-up. <br />Called and discussed cleanup scheduling with Ash Moghaddam with Patriot <br />Environmental Services. Clean up is scheduled for 1/13 at 8am. Called and left Debra a voice message. Emailed Debra notifying her that <br />I will be out of the office 12/18-12/26 and 12/29-1/4, so if the clean-up will be <br />performed while I'm away, please call our main line and ask for our on-call <br />inspector to verify the clean-up is complete (noted that our main line is dispatched <br />to our county hospital after 5pm and on weekends). Reminded Debra that we <br />need to verify the clean-up prior to backfill of any excavations.
