Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COXFM5K50 Site Location: 8842 W SCHULTE RD <br />Activity Summary <br />Activity Date Recorded by <br />11/25/2021 Called Dave Larson (PG&E) for scheduling a clean up at 7 am. Dave L indicated ERNEST <br />clean up will occur in the coming hours, that currently they are performing the road <br />closure. <br />Called Dave Larson at approximately 9:00 am asking for an update, and giving <br />Dave L. a secondary contact number due to poor cell coverage in my area. Dave <br />L. indicated cleanup should be completed in 2 hours. <br />Called Dave L. at approximately 1:00pm. He indicated to drive to the cleanup site. <br />Traveled to 8842 W Schulte Rd in Tracy for a clean up of mineral oil. Made <br />contact with Dave Larson. Took photos of the site. Discussed lab results, sent to <br />John Alaniz. Indicated lab results showed the PCB content to be a non detect. <br />Waste is to be taken to general waste, if possible on 11/25. If not waste will be <br />taken back to facility yard where disposal would occur on 11/29. <br />11/29/2021 Emailed Alex Steele, <R25v@pge.com>, asking for the PCB results. ALANIZ <br />11/29/2021 Discussed cleanup with Fred Saeed. One 20 yard roll-off bin of waste was ALANIZ <br />generated. <br />12/01/2021 Called Alex Steele and left a message asking for the PCB results and an update to ALANIZ <br />the status of the waste. <br />12/01/2021 Discussed photo log creation, and site disposition with John Alaniz. Discussed ERNEST <br />scheduled disposal times for the dumpster used for clean up. Moved photos from <br />phone to computer. Started photos log. <br />12/02/2021 Received a photo log for the final cleanup from Fred. Filed with the compliant. ALANIZ <br />12/02/2021 Mailed a copy (inter department mail) of the prop 65 to Road Maintenance. ALANIZ <br />Called Alex Steele and confirmed that the waste had been taken to 4040 West Ln <br />after cleanup had been completed. Alex also said he would email me a copy of <br />the PCB results. <br />12/02/2021 Discussed photo log with John Alaniz. Finished photo log, converted photo log to a ERNEST <br />pdf. Emailed John Alaniz photo log. <br />12/09/2021 Sent an email to Alex asking for information on the "blanket" encroachment ALANIZ <br />permit. <br />Called Alex and asked for a copy of the PCB results. <br />12/16/2021 Called and emailed Alex Steele asking for the PCB results and information on the ALANIZ <br />"blanket" encroachment permit. <br />12/17/2021 Received PCB results from Alex Steele. Results came back as non-detect. ALANIZ <br />Started to abate compalint. <br />12/17/2021 Corrected dailies and sent an email to Fred Ernest to correct a daily.
