The conditions imposed by the Local Health District were made a part <br />of the approval: <br />1. This tentative division of land shall meet the Rules and Regulations of <br />San Joaquin County Ordinance #549. A percolation test and a minimum <br />sewage disposal area are required for Local Health District approval. <br />A permit for the percolation test from the Health District is required. <br />2. The existing wells on Parcel #6 must be destroyed under permit and <br />inspection of the Local Health District. <br />3. An "Improvement Plan" is required to be approved prior to the recording <br />of the final map and shall include the following: <br />a. Location of sewage disposal area on each parcel. <br />b. Location of well site on each parcel. <br />c. Location of all existing septic systems and wells within <br />150 feet of the proposed subdivision. <br />Since approval of this permit included San Joaquin Local Health District conditions, <br />you are hereby advised that the conditions shall be met prior to utilization of the <br />approved land use. <br />If the decision is appealed to the Board of Supervisors, the filing of the appeal <br />stays the issuance of this permit or any other permits until the appeal has been <br />decided by the Board. <br />If you have any questions req,,rdiml thit. matter, call (ZD,i) 466-6781. <br />Jogi Khanna, M.D., M.P.H. <br />District Health Officer <br />Ron L.
