The hazardous work policy shall be available for inspection by employees and contractors. <br /> II. GENERAL REQUIREMENTS <br /> A. A hazardous work permit is valid only for the specific work described on the permit. No <br /> other work is to be performed regardless of the proximity or similarity to the approved <br /> hazardous work. Additional permits must be issued for other work requiring a permit. <br /> B. A hazardous work permit is made out to the employee(s) or contractors who will perform <br /> the permit work and shall not be valid for anyone else. Revalidation of each permit <br /> requirement must be made for each change in personnel. Personnel must sign their names <br /> on the permits. The area in proximity to the hazardous work shall be identified and <br /> isolated using red danger tape or other form of clearly labeled barricade to prevent anyone <br /> from entering the hazardous work area. Only persons who are signed in on the Hazardous <br /> Work Permit are allowed to enter the taped off area.
