Intermediate <br /> bleed valves, installed in conjunction with single block valve arrangements, should be <br /> used wherever available to control the venting, draining, and inert purging of affected <br /> process piping. Work on natural gas or other fuel-based process piping systems are also <br /> defined as a level two hazard. Penetration into the process pressure boundary of fuel-based <br /> systems requires inert purging through the affected work area using sufficient equivalent <br /> of volumetric changes. A job planning meeting must be conducted to review hazards and <br /> precautions prior to permit authorization for all level two permits. <br /> Level 3 Hazardous Work Definition—Any time that a hot work permit is required in <br /> conjunction with a hazardous work permit the work is considered to be level three. Any <br /> work on HTF systems when the temperature of the line or the temperature of the fluid <br /> anywhere in the system is above 140 F requires a level three permit. A job planning <br /> meeting must be conducted to review hazards and precautions prior to permit <br /> authorization. <br /> E. A hazardous work permit shall only be issued and authorized when the work area meets <br /> the following requirements: <br /> 1. Hazardous work will not be permitted at the same time in areas where hot work is <br /> authorized for other work within 50 ft. of the hazardous work site(including <br /> overhead). <br /> 2. Hot work in conjunction with a hazardous work permit can only be completed with <br /> the approval of the Plant Manager or Engineering Manager. Welding or cutting <br /> shall only be permitted on drained sections of the process system after inspection <br /> by the Engineering or Plant Manger. All exceptions must be approved by the <br /> location Plant Manager. He shall show his approval by initialing the hazardous <br /> work permit. All exceptions shall be shown under "Special Precautions and <br /> Limitations" on the hazardous work permit form. <br /> 3. No spark-producing grinding,cutting, or other work activities which create <br /> ignition sources will be permitted within 50 ft. of the hazardous work site that is <br /> not already approved as part of the Hazardous Work permit(level 3). <br /> 4. No hazardous work is allowed in an area where the sprinkler system is out of <br /> service. The Engineering Manager or Plant Manager may authorize exceptions to <br /> this after taking the necessary precautions. <br /> 5. The section of the process system to be opened shall be made safe by positive <br /> isolation from the flow of material. Isolation is to be provided by: (1) first closing, <br /> locking. and taasina the anpronriate block valves both upstream and downstream <br /> of the point of opening; and(2)locking open the appropriate drain valve in the <br /> three-way valve system (where applicable), or(3) inserting blinds rated for full <br /> line pressure with tags at the pipe flanges. <br />
