Shall inform the employee of any special personal safety equipment,precautions, and <br /> limitations shown on the permit. <br /> D. Shall instruct employees that hazardous work can only be performed when they have a <br /> permit,which has been completely filled in and approved. <br /> E. Be responsible for knowing the scope of the job, all potential job hazards, and for <br /> providing special tools and safeguards required. <br /> F. Ensure that the means of summoning the required emergency service is operable. <br /> G. Review and assure by questions and answers that all workers understand their duties and <br /> required procedures for permit work. <br /> H. Sign the permit at the Lob site, signifying all necessary permit conditions are met, <br /> including proper completion of the permit. <br /> 1. Must provide normal levels of control, inspection, and direct supervision to ensure safe <br /> completion of the work. <br /> VI. AUTHORIZED WORKERS RESPONSIBILITIES <br /> An authorized worker is an employee certified by training to perform hazardous work and is <br /> authorized to perform permit work as shown on the hazardous work permit form. This is <br /> typically trained operators,maintenance personnel and engineers. <br /> AUTHORIZED WORKER SHALL: <br /> A. Initiate and fill out the required permit. (Only Authorized employees, hazardous work <br /> trained workers or supervisors are permitted to issue a Hazardous Work Permit) <br /> B. Prepare the work area and determine that no hot work or grinding is being performed <br /> within a 50 ft.radius (including overhead). <br /> C. Contact his/her supervisor and the operating area supervisor for perinit approval when all <br /> preparations have been completed.Notify management of such work. <br /> D. Complete all work safely. A high temp face shield and leather gloves in addition to <br /> standard PPE will be worn while performing hazardous work. The condition of the system <br /> to be opened must be evaluated prior to breaking in. In the event that the system has a <br /> potential for built up pressure the employee(s)breaking into the line must wear the <br /> protective hot suit(aluminized coat or apron). The Operating Area Supervisor will <br /> determine if this extra precaution is required. <br /> E. Contact the authorizing Operating Area Supervisor upon completion of the hazardous <br /> work to have the permit closed out. <br />
