All employees shall immediately leave and stay out of any vapor clouds and notify ERT of <br /> vapor cloud. <br /> G. Keep the amount of material drained from the system to a minimum. <br /> H. Must keep the authorized (signed)permit at the work site. <br /> 1. Know the potential hazards of the permit work and the appropriate procedures to minimize <br /> them. <br /> J. Duplicate of D <br /> K. Print and sign his/her name on the permit prior to beginning work, signifying his/her <br /> agreement with the safety precautions and the understanding of the hazards. <br /> L. Know the proper procedure to notify the facility of an emergency <br /> M. Summon emergency services as soon as any conditions develop which may require <br /> assistance (fire,burns, etc.) <br /> N. Take the following actions when unauthorized persons approach the permit work area <br /> while work is underway. <br /> 1. Warn the unauthorized person that they must stay away from the permit work area. <br /> 2. Cease all permit work until the unauthorized person leaves the area. <br /> VII. OPERATING AREA SUPERVISOR RESPONSIBILITIES <br /> The authorizing operating area supervisor is an employee who is responsible for the operating <br /> area at the time the permit work is to be accomplished. The operating area supervisor is certified <br /> by training and inspects the hazardous work area for compliance to this hazardous work program. <br /> The Operating Area Supervisor must determine the hazard level of the work being completed <br /> under the permit. If the level of hazard is identified to be a level 2 or level 3, the Operating <br /> Supervisor must organize a planning meeting and acquire approval of the appropriate member of <br /> senior management. <br /> OPERATING AREA SUPERVISOR SHALL: <br /> A. Determine that the hazardous work represents the only reasonable means of completing <br /> the work and that permit work is kept to a minimum. <br /> B. Inspect the item to be repaired or replaced to determine that all conditions and safeguards <br /> have been completed prior to issuing a permit. <br /> C. Conduct planning and review meeting for all level 2 and level hazardous permit <br /> activities prior to signing the permit. <br />
