Issues identified in the review will be addressed and communicated <br /> to the plant manager and employees. <br /> C. Hazardous work permits shall be retained for a minimum period of year. <br /> XI. HTF SYSTEM OUTAGE <br /> Circulating and maintaining the temperature of the Heat Transfer Fluid is critical to plant operations. At <br /> times it may be necessary to take the system out of operation for a planned outage or an unplanned event <br /> such as a power outage, gas supply problem, leak in the system, or process control problems, etc. <br /> A. Planned Outages <br /> Any time that there is a scheduled shutdown for planned maintenance or improvements to the system a <br /> planning meeting must be conducted to review the work to be performed,identify potential hazards and <br /> determine required precautions to complete the work safely. <br /> The scope of work is to be clearly defined and agreed to by maintenance, engineering,production, and <br /> process engineering. This scope is to include but is not limited to a description of the work, safety <br /> requirements,parts and tools needed,preparation work,effects on other plant areas, outside resources, <br /> vendors, and a back up plan to deal with unforeseen problems. A listing of any permits that are required <br /> should be developed in advance so that completion of permits at the job site prior to work beginning can <br /> be efficiently done.. <br /> A schedule of events is required. This schedule must show the person responsible, manpower <br /> requirements,progress checkpoints, and relationship of this work to other jobs in the plant. The HTF <br /> system should not be out of service for more than three hours without a plan to monitor system <br /> temperature, and component conditions. Any work beyond three hours must be planned so that the <br /> system can be put back into service at any half-hour interval. Supervision must be provided with an <br /> hourly update, so a decision can be made to continue or stop work. <br /> B. Unplanned Outages <br /> In the event of an unplanned outage where maintenance work must be completed to restore circulation of <br /> the HTF system, the level of the permit work required to be complete to must be determined. In the event <br /> that level 2 or level 3 hazards will be encountered,proper approvals for hazardous work permits must be <br /> acquired before the work can be initiated. Emergency situations can not preempt the hazardous work <br /> permit procedures. In an emergency situation where management is not immediately available on site, <br /> they must be contacted for verbal authorization prior to level 2 or level 3 hazardous works being initiated. <br />
