Verify that emergency services and equipment are available and the means for <br /> summoning the service are operable. <br /> 3. Authorize permit work. <br /> 4. Oversee permit work operations. <br /> 5. Terminate permit work. <br /> 6. Debrief the contractor at conclusion of his employee's hazardous work operations. <br /> C. Workers shall be trained in: <br /> 1. Hazard recognition such that the worker can determine if conditions are <br /> acceptable. <br /> 2. Proper procedure to alert emergency services. <br /> 3. Protective equipment required its correct use, and determination that PPE is in <br /> satisfactory condition. <br /> D. The required annual hazardous work permit program training shall be certified. The <br /> certification shall contain: <br /> 1. Each trained employee's name and signature <br /> 2. Signature of the trainer <br /> 3. Dates of the training <br /> 4. Training presented which shall include Responsibilities of the authorized <br /> employees,how to properly complete hazardous work permits and the hazard <br /> assessment process. <br /> E. The hazardous work permit program training records shall be filed by Human Resources <br /> department. <br /> X. REVIEW,REVISIONS AND RETENTION <br /> A. The hazardous work permit program and permit form shall be reviewed and revised <br /> whenever there is reason to believe that measures taken may not protect employees during <br /> permit work. <br /> B. All hazardous work permits issued shall be reviewed upon completion of the job. Review <br /> of completed permits shall occur at the daily operations meeting on the day following the <br />
