5.0 SAFETY MEETINGS <br />5.1 Safety Briefing <br />Before on-site work commences, all PSC and subcontractor employees assigned to work on the site <br />must be briefed by the SSO (or his designate) on the site-specific health and safety requirements <br />contained in this plan, as well as the job functions, potential hazards present, and the protective <br />equipment required. The person giving the briefing should test the workers knowledge and <br />understanding of the provision of this HASP and shall not allow anyone who does not appear to <br />understand the provisions perform work in the exclusion areas. The standard PSC JSA will be <br />completed and signed by all personnel working the site. <br />5.2 Distribution of HASP <br />Before the work begins, a copy of this HASP must be available for review by each PSC employee and <br />subcontractor employee assigned to work at the site, as well as authorized representatives of each firm <br />contracted by PSC to perform work on site. The Project Supervisor will maintain the following records <br />and/or copies of records on-site: <br />♦ A list of personnel qualified to work, in the form of a sign -in sheet of all personnel working <br />at the site. <br />♦ This Health and Safety Plan. <br />♦ Training certifications for all personnel working the HHW Collection Event showing dates <br />of 40 -hour and annual refreshers. <br />• For personnel conducting activities that require respiratory protection, proof of fit testing. <br />♦ Exposure monitoring data, as appropriate. <br />5.3 Incident Reporting <br />Injuries, exposures, illnesses, safety rule infractions, and other incidences must be reported <br />immediately or, at the latest, within 24 hours of occurrence. <br />5.4 Visitor Clearances <br />Visitors will not be allowed within the perimeter of the decontamination and exclusion zones. <br />5.5 Posting Requirements <br />The following information shall be posted or readily available on site: <br />♦ Emergency phone numbers <br />♦ Directions to the nearest hospital <br />♦ Health & Safety Plan <br />5.6 Local Agencies <br />Provisions have been made with the local agencies and emergency responders which have been <br />briefed on the specifics of the facility operations and emergency action plan, including information <br />on the nature of the hazardous materials that are handled at the site, traffic control measures, site <br />maps, evacuation procedures, and this facility's emergency action plan. Phone numbers for local <br />agencies and emergency responders are listed in section 8.3. <br />Local Emergency Responders Contact: Upon implementation of the emergency action plan, by dialing <br />911 the local agency responders --Lodi Fire Department, Lodi Police Department, County Office of <br />Emergency Services, and/or paramedics-- will be notified and respond to the incident as appropriate. <br />MSDS information shall be provided whenever feasible in the event of a chemical spill or exposure. <br />Local Community Hospital Contact (See Hospital Map): The Lodi Memorial Hospital is closest to the site <br />and has a 24 hour — 7 days a week Emergency Room. In the case of chemical exposure an MSDS for <br />the substance shall be obtained and provided to the treatment facility. The address, phone numbers, and <br />Hospital Map with directions for traveling from the THHWCF site to the medical facility are provided in <br />Section 8.4 on Page 15 of this plan. <br />7 <br />
