Reporting requirements are identified in <br />Section XVI of the General Permit and include (but are not limited to) providing a summary of: <br />1) Corrective actions and compliance activities, including those not implemented; <br />2) Violations of the General Permit; <br />3) Date, time, place, and name(s) of the inspector(s) for all sampling, inspections, and field <br />measurement activities; <br />4) Visual observation; and <br />5) Training documentation of all personnel responsible for General Permit compliance <br />activities. <br />The contractor may include copies of the annual reports in Appendix L <br />SECTION 1.8 - CHANGES TO PERMIT COVERAGE <br /> <br />The General Permit (Section II.C) allows a permittee to reduce or increase the total acreage covered <br />under the General Permit when a portion of the project is complete and/or conditions for <br />termination of coverage have been met; when ownership of a portion of the project is sold to a <br />different entity; or when new acreage is added to the project. <br /> <br />To change the acreage covered, the permittee must electronically file modifications to PRDs (revised <br />NOI, site map, and SWPPP revisions) and a certification that new landowners have been notified of <br />applicable requirements to obtain permit coverage (including name, address, phone number, and e- <br />mail address of new landowner) in accordance with requirements of the General Permit within 30 <br />days of a reduction or increase in total disturbed area. Include any updates to PRDs submitted via <br />SMARTS in SWPPP Appendix E. Document any related SWPPP revisions/amendments (Section <br />II.C.2) in SWPPP Appendix C. <br />SECTION 1.9 - NOTICE OF TERMINATION <br /> <br />To terminate coverage under the General Permit, a Notice of Termination (NOT) must be <br />submitted electronically via SMARTS. A “final site map” and photos are required to be submitted <br />with the NOT. Filing a NOT certifies that all General Permit requirements have been met. The <br />NOT may be submitted if one of the following options applies and all underlying conditions are <br />met: <br /> <br />1) Construction project is complete and the following conditions have been met: <br />a. All elements of the Storm Water Pollution Prevention Plan have been completed <br />b. Construction materials and waste have been disposed of properly <br />c. The site is in compliance with all local storm water management requirements <br />d. A post-construction storm water operation and management plan is in place <br /> <br />2) Construction activities have been temporarily or indefinitely suspended, and the following <br />conditions have been met: <br />a. All elements of the Storm Water Pollution Prevention Plan have been completed
