Time In: 1:48 pm <br /> Time Out: 2:20 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: COUNTRY KITCHEN Date: 05/26/2022 <br /> Address: 1327 W LOCKEFORD ST, LODI 95240 <br /> Owner/Operator: XENOS, DENNIS T Telephone: (209)368-6350 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00054858 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> HOOD IN THE KITCHEN WAS PAINTED BLACK AND NOW IT'S CHIPPING OFF THE HOOD ONTO THE FOOD. NEW <br /> PIPING IN CEILING IS BLOWING PARTICLES INTO THE AIR AND ONTO THE FOOD. THE ICE MAKER IS UP ON <br /> BLOCKS AND THE WATER BACKFLOWS INTO THE MACHINE. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I went to the facility and spoke to the owner of the facility(who took over May 15, 2022). <br /> 1 observed a section of the cooking hood that is peeling. <br /> Repaint or provide so that surface is smooth, cleanable and non absorbent within 1 week. <br /> Facility is not open at this time(Closes at 2pm) <br /> I did not observe"New piping in ceiling that is blowing particles into the air and onto the food"at this time. <br /> I observed ice maker and did not observe water"back flowing into the machine" <br /> Operator to provide an air gap for discharge pipe for ice machine within 1 week. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: Abdul, <br /> EH Specialist: STEPHANIE RAMIREZ Phone: <br /> FA0000424 C00054858 SCO04 05/26/2022 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 1 Food Program Complaint Inspection Report <br /> 1868 E.
