SAN-JOAQUIN <br />---COUNTY <br /> Greatness grows here. <br />Environmental Health Department <br />Time In: 10:28 am <br />Time Out: 11:00 am <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: PANERA BREAD #968 Date: 05/03/2022 <br />Address: 2533 KETTLEMAN LN, LODI 95242 <br />Owner/Operator: PANERA LLC Telephone: (209) 367-1322 <br />Program Element: 1600- FOOD PROGRAM Complaint #: 000054845 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT ORDERED A NAPA CHICKEN SALAD SANDWICH 0N4/20/22 AT AROUND 1:30 PM AND ATE IT AT <br />AROUND 2:00 PM. AROUND 5:30 PM COMPLAINANT BEGAN TO EXPERIENCE SEVERE GUT PAIN. BY 6:00 PM <br />COMPLAINANT BEGAN TO EXPERIENCE VOMITING, CHILLS, AND GUT PAIN INTENSIFIED. STOMACH SWELLED <br />UP AND COMPLAINANT WENT TO ER AROUND 1:30 AM COMPLAINANT GOT CT SCAN AND DIAGNOSED WITH AN <br />INFLAMED COLON. COMPLAINANT STATES THAT THEY DID NOT RESPOND TO ANTIBIOTICS SO DOCTORS <br />PRESCRIBED THEM AN ANTI FUNGAL AND COMPLAINANT DID A LITTLE BETTER. COMPLAINANT BELIEVES <br />THERE MUST HAVE BEEN SOME SORT OF MYCOTOXIN OR TOXIC MOLD IN THE IN THE SOURDOUGH BREAD. <br />COMPLAINANT IS STILL EXPERIENCING SYMPTOMS SIX DAYS AFTER CONSUMING SANDWICH. <br />***PLEASE CALL COMPLAINANT WITH FINDINGS. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION --TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />COMMENTS AND OBSERVATIONS <br />I spoke to the manager who was aware of the complaint prior to this investigation. <br />Napa Chicken salad sandwich is made by the following ingredients: <br />2 bags of packaged cooked chicken is mixed with Panera salad dressing <br />these items are kept in the walk-in (39F) <br />Chicken mixture is then mixed with pre cut, pre washed bagged celery. <br />Grapes are added to the mixture. <br />Grapes are washed and cut on site. <br />Pre packaged almonds are added to the mixture as well. <br />All of the above food items are kept in the walk-in prior to use. <br />Once mixed, the mixture is placed on the sandwich bar line. <br />Tomatoes and lettuce are added to the sandwich. <br />Sourdough bread is made daily. <br />Every day at closing whole loaves of unused sourdough bread is donated, and sliced sourdough bread is disposed. <br />I observed sandwich assembly. <br />Food handler washed hands and put gloves on prior to assembling sandwich. <br />I observed sourdough bread. <br />I did not observe mold in bread. <br />FA0017683 C00054845 SC004 05/03/2022 <br />EHD 16-23 Rev. 04/07/22 <br />Page 1 of 2 Complaint Inspection Report <br />1868 E.
