Complaint Investigation Summary <br /> Report #. 5105 <br />COMPLAINT ID: C00054037 Site Location: 12614 N JACK TONE RD <br />Activity Summary <br />Activity Date <br />10/25/2021 Discussed complaint with lead senior and program coordinator. Additional <br />property owner information is needed. Received assistance from program <br />coordinator Steven Shih to look up recent demo permits for the property. No <br />additional information for the property owner was available. I looked in megabyte <br />and confirmed the property owner is Roxana Tapia. I generated a notice to abate <br />and mailed it to 1708 Pawtucket Ave, Modesto, CA 95358. I also emailed Corrine <br />King of Community Development for additional information regarding this <br />complaint. <br />Recorded by <br />VANG-LEE <br />10/26/2021 Mailed complaint cover letter - SECOND NOTICE - dated 10/25/21 by regular mail MARTINEZ <br />to: <br />ROXANA TAP IA, <br />1708 PAVVTUCKET AVE, <br />MODESTO CA 95358. <br />10/27/2021 The invoice was returned to accounting as "not deliverable as addressed unable to VAN G-LEE <br />forward". I provided accounting with the following mailing address: 1708 <br />Pawtucket Ave, Modesto, CA 99535. <br />10/29/2021 Received an invoice back in the mail. Looked up a the property on Oasis and saw SUSZYCKI <br />that the owner's address has changed. <br />Submitted the new address to accounting. <br />11/15/2021 Emailed Steve Thomas of code enforcement to get an update of the complaint on <br />their end. Steve stated that their case is still open for building issues and he still <br />has not made any contact with the property owner. Steve states that he drove by <br />the property and the outside appeared to be clean. <br />05/17/2022 I emailed Steve Thomas of Community Development and was provided a contact <br />number for the property owner Roxana Tapia, 650-346-0335. Per Steve, he does <br />not recall observing any hazardous materials on site the last time he was there. I <br />called and left a message for Roxana requesting for a call back. <br />VANG-LEE <br />VANG-LEE <br />05/25/2022 I returned a call to Roxana Tapia at 650-346-0335 and left a message. VANG-LEE <br />05/31/2022 I contacted Roxana Tapia at 650-346-0335. I asked Roxana what happened to <br />the materials observed on site during our inspection last year. Roxana states that <br />she was informed by the person in charge of caring for the residence that there <br />were no materials on site. I informed Roxana that we have documentation and <br />photos verifying materials were observed at the time of our inspection. Roxana <br />states she will need to get more information regarding the materials and get back <br />to me. I emailed copies of the inspection photos and notice to abate to Roxana <br />via email at roxanatm12@gmail.com per her request. <br />06/08/2022 I received an email from property owner Roxy Tapia <roxanatm12@gmail.com> <br />stating that she was informed by the landlord, Ignacio, that the renters took all the <br />materials with them. They do not know what the renters did with everything. <br />I updated the complaint record with the property owners mailing address and <br />phone number.
