0 isposal Site Standar,* Page 2 <br />CoTfi-pliance Statements_ For D <br />Comply with all circled violations: <br />1E5D8- Adequate barriers were not provided around the perimeter of <br />the disposal site. <br />17659 - Access roads to the disposal site were not smooth and designed <br />to minimize the generation of dust and the tracking of mud <br />during inclement weather conditions. <br />(17660 - Internal roads used by the public or commercial vehicles <br />were not maintained in a safe condition or maintained such <br />that vehicle access and unloading can prbceed safely during <br />inclement weather conditions. <br />17666 Sanitary Sanitary facilities were not available at the site or not <br />maintained in a clean usable condition. <br />EiD- Safe and adequate drinking water was not provided for the <br />site personnel. <br />17668 -- Communication facilities were not available to the site <br />personnel. <br />1,7669 - The site and equipment were not provided with adequate <br />lighting while operating after dark. <br />Approved safety helmets and respirators were not provided for <br />employees operating equipment at the site. <br />Employees operating •equipment at the site were not using the <br />approved safety equipment provided by the operator. <br />17676 - The unloading area was not confined to a small enough area <br />at the site to provide adequate control of . all wind blown <br />materials. <br />17677 - Refuse was not spread in compacted cells not to exceed a <br />depth of approximately two (2) feet before compaction. <br />17678 - Slopes and cuts of the working face were not maintained at <br />a ratio which allows effective compaction of the wastes. <br />17679 - The slope of the portions' of the fill which will be the <br />final exterior surface was not finished with a final cover <br />o <br />that has a neat appearance and a horizntal to vertical ratio <br />not to exceed one and three quarters to one (1). <br />17680 - Stockpiled cover material was placed in an area that inter- <br />fered with the operation of the site. <br />17681 - Sufficient quantity of cover material was not available to <br />meet the requirements of the site operations. <br />I I A <br />
