' <br /> � w <br /> ATTACHMENT A <br /> Hazardous Waste Facility Permit <br /> Forward, Inc. <br /> Stockton, California <br /> \ . <br /> GENERAL PK0V\S(0N3 <br /> l . Forward' Inc. , hereinafter called the operator, shall comply with the <br /> . provisions of the Health and Safety Code, including Chapter 6.5 of <br /> Division 20, and with the Minimum Standards for Management of Hazardous <br /> and Extremely Hazardous Wastes (Chapter 30, Division 4^ Title 22 of <br /> the California Administrative Code) . <br /> 2. The operator shall ensure that the operation of the facility will not <br /> Imperil public health and safety, wildlife, domestic livestock, or <br /> the environment. <br /> 3. The operator shall allow the California State Department of Health <br /> Services or the San Joaquin Local Health District to inspect the <br /> facility/ take samples of wastes, and Inspect pertinent records. <br /> 4. The operator shall report to the California State Department of Health <br /> Services within 24 hours after occurrence all accidents involving ' <br /> hazardous wastes which resulted In, or could have resulted in, a <br /> hazard to public health and safety, wildlife, domestic livestock, or <br /> to the environment. <br /> 5. The opurator shall maintain the qualified personnel and the equipment <br /> necessary to provide for the safe operation of the facility. <br /> 6. The operator shall notify the California State Department of Health <br /> Scrviceu of a proposed change in ownership of the facility, of substan- <br /> tial changes in the facility or In the method of operation of the <br /> facility, or of proposed closure of the facility 30 days prior to such <br /> event. <br /> 7. The operator shall : <br /> (a) File this ponn/t at the Forward Hazardous Waste Facility or at <br /> his place of business; <br /> /b\ Require that a manifest be completed for all hazardous wastes <br /> accepted , send legible copies to the California State Department <br /> of Health Services on a monthly basis, and retain records for a <br /> period of on: year; and <br /> (c) Pay foes for disposal of hazardous waste In conformance with <br /> Article 3, Chapter 30, Division 4, Title 22 of the California <br /> Administrative Code, <br />
