The signs shall indicate the <br /> entrances and exits, the traffic pattern, the hours and days <br /> when the facility Is open, the types and quantities of wastes <br /> whIch' can be acccpted' and the name and telephone number of <br /> the person to be contacted /n an emergency. <br /> l. Within 60 days' of the date of this permit, the operator shall : <br /> /a\ Have completed installation of all required fencing and gates; a»d <br /> (b) Make available to all persons on-site a sink, soap, and water <br /> for c|can-up, - <br /> 4. By the 15th day of 1ho month following each calender quarter, the <br /> operator shall submit to the California State Department of Health <br /> Services a report of analysis of groundwater from each monitoring <br /> well (monitoring wells designated by the California Regional Water <br /> Quality Control Board, Central Valley Region, shall he used) . The <br /> report shall include, but not be limited to, date of sampling, ele- <br /> vation of water in the well , and concentrations of arsenic, barium, <br /> strontium` ch\or7des, and sulfate in the water. <br /> 5. As part -of the operation plan required in Part i Scct /on I of this <br /> permit, 'the operator shall prepare a plan for the sampling and analysis <br /> of soil , grasses, and crops representative of the area within a 500- <br /> foot radius of the fence surrounding the hazardous waste area of the <br /> facility. By the 15th day of the month following each caien6ar quarter <br /> the operator shall submit to the California State Department of Health ' <br /> Services a report which shell Include, but not be limited to, the date <br /> of sampling , the location of sampling, and the total concentrations of <br /> arsenic, barium, and strontium In each sample. A grid map that Iden- <br /> tifies where each sample was taken shall also be included. Wherever <br /> applicable, the operator shall secure the permission of landowners <br /> adjacent to the facility prior to obtaining samples for analysis. <br /> 6. 'The operator shall : <br /> (a) Notify the California State Department of Health Services 30 <br /> days in advance of the opening of any new pit or other <br /> excavation Intended for disposal of hazardous waste; and <br />
