Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />August 25, 2022 <br /> Facility Address: <br /> 3412 Perlman Dr, Stockton <br /> Facility Name: <br /> O'Reilly Auto Parts DC #23 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete since the following item is not on the map: <br />1. Lead acid automotive battery core returns were stored in the Returns Receiving area (northeast section of the <br />warehouse) but their storage location is not on the map. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. If a site map element is not applicable <br />for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information was not reviewed and submitted in the California Environmental <br />Reporting System (CERS) annually by the due date, January 15 for the 2020 reporting year. The 2020 CERS <br />submittal was done on 1-24-20. <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: Please provide a corrective action statement to the San Joaquin County Environmental <br />Health Department. <br />This is a repeat violation, Class II. <br />FA0020539 PR0535615 SC001 08/25/2022 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 2 of 3 <br />1868 E.
