ALLEGEDLY COMPLAINANT'S BROTHER HAD AN ALLERGIC <br />REACTION TO FRENCH FRIES AFTER SHRIMP WAS COOKED IN THE SAME FRYER, WHICH RESULTED IN TRIP TO <br />E. R. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately Non-compliance may warrant immediate closure of <br />the food facility. <br />#1 Demonstration of Knowledge <br />OBSERVATIONS: The food manager's certificate for Hadram Ali Alhadrami expired on April 10, 2022. Provide proof of <br />renewal of the 5-year food manager's certificate to lbaker@sjgov.org. Correct within 14 days. <br />CALCODE DESCRIPTION: All food employees shall have adequate knowledge of and be trained in food safety as it relates to their <br />assigned duties. (113947) Food facilities that prepare, handle or serve non-prepackaged potentially hazardous food, shall have an <br />employee who has passed an approved food safety certification examination. (113947-113947.1) Any food handler hired after June 1, <br />2011 shall obtain a Food Handler Card within 30 days (113948). <br />#35 Equipment/Utensils Approved and in Good Repair <br />OBSERVATIONS: One fryer is non-operational. Repair/replace fryer within one month. <br />CALCODE DESCRIPTION: All utensils and equipment shall be fully operative and in good repair. (114175). All utensils and equipment <br />shall be approved, installed properly, and meet applicable standards. Hadram Ali Alhadrami (Owner) was aware of the complaint. Hadram Ali Alhadrami stated that if the <br />complainant notified the staff that there was an allergy they would have advised them not to eat there or accommodated the <br />request if they could. Food handler cards were available for review. The food manager certificate was expired. Per Hadram <br />Ali Alhadrami, if a customer notifies them that there is a food allergy, all employes are trained to advise the customer to not <br />eat there. Accommodations will be made if possible. There are currently two fryers but one fryer is broken.
