PERMIT NO.: 1022 -SSV -0147 <br />PERMITTEE: Ground Zero Analysis, Inc. <br />DATE: March 9, 2022 <br />"Your attention is directed to Standard Specification, Section 5-1.36 Property and Facility <br />Preservation, and Business and Professions Code, Section 8771. Permittee shall physically <br />inspect the work site and locate survey monuments before work commencement. Monuments <br />that might be disturbed shall be referenced or reset in accordance with Business and Professions <br />Code." <br />"If feasible, monuments should not be set within the traveled way. All monuments that must be <br />set or perpetuated in paved surfaces, shall be constructed in accordance with Caltrans Standard <br />Specification Section 81 'monuments' and Standard Plan A74, Type D, or equal with prior <br />approval of the District Surveys Engineer." <br />"Copies of Corner Records filed or Record of Surveys recorded in compliance with the Business <br />and Professions Code shall be forwarded to the District Surveys Engineer." <br />Except when necessary, as determined by the State Representative, no installation, construction <br />equipment or personal vehicles shall operate or park within the traveled way. <br />All work shall be conducted in such a manner that the excavation, excavated earth, materials and <br />equipment will not cause any inconvenience to the highway traffic or to traffic entering the highway <br />from any public or private approach. <br />No earth or construction materials are to be dragged or scraped across the highway pavement. <br />No excavated earth shall be placed or allowed to remain at a location where it can be tracked <br />onto the highway traveled way or any public or private approach by the Permittee's construction <br />equipment, or by traffic entering or leaving the highway traveled way. Any excavated earth or <br />mud so tracked onto the highway pavement or public or private approach shall be immediately <br />removed by the Permittee. <br />All Caltrans underground infrastructure including but not limited to traffic loops and <br />electrical conductors must be marked in the field before work begins. <br />Any damage to State property caused by any authorized activity by this permit shall be <br />immediately repaired and brought to its original condition or better at no cost to State to the <br />satisfaction of the State Representative <br />Caltrans is not a member of USA Underground Service Alert (underground locating service) <br />and USA does not locate Caltrans underground facilities. It is the responsibility of the permittee <br />to verify location and depth of all existing, underground Caltrans facilities including but not limited <br />to drainage facilities, traffic loop detectors, electrical conduits, pull boxes and any other electrical <br />facilities before using tools or equipment that may damage those facilities or interfere with an <br />electrical system. Permittee shall replace, in kind or better, all existing electrical and non- <br />electrical facilities damaged during construction. <br />Permittee is responsible to identify all existing Caltrans underground facilities. The facilities shall <br />be marked in the field and depth of cover of each facility should be identified. A copy of all the <br />findings should be given to the state representative. No work can begin until all Caltrans <br />underground facilities are identified and marked in the field and there are no conflicts with all the <br />existing Caltrans underground facilities. <br />4of6 <br />
