PERMIT NO.: 1022.6SV.0147 <br />PERMITTEE: Ground Zero Analysis, Inc. <br />DATE: March 9, 2022 <br />PEDESTRIAN SAFETY: when the work area encroaches upon a sidewalk, walkway, or crosswalk <br />area, special consideration must be given to pedestrian safety. Protective barricades, fencing, <br />handrails, and bridges, together with warning and guidance devices and signs must be utilized so <br />that the passageway for pedestrians, especially blind and other physically handicapped is safe <br />and well defined and shown on the approved permit plan. <br />In addition to the attached General Provisions, the following special provisions are also applicable: <br />1. Unless specifically authorized elsewhere in this permit, survey work shall not be performed: <br />A. On holidays of an afternoon proceeding a holiday. <br />B. In medians. <br />C. When work requires traffic control on traveled lanes. <br />2. Permittee shall furnish, place, and maintain required signs, safety equipment, traffic control, <br />and waming devices in accordance with Caltrans Manual on Uniform Traffic Control Devices <br />2014. <br />3. All personnel shall wear hard hats and class 3 vests, shirts or jackets as appropriate. <br />4. Unless specifically authorized in this permit, survey markings within the right of way shall be <br />temporary. Paint used for markings shall be water-soluble and other markers shall be <br />removed upon completion of surrey. <br />5. Any collected survey data requested by Caltrans shall be furnished to Caltrans without charge. <br />6. Measurements across traffic lanes shall be made through the use of an electronic distance - <br />measuring device utilizing non-visible light or other optical means. <br />7. Electromagnetic and radioactive equipment must be operated by certified personnel and shall <br />not interfere with radio communications or be directed toward the traveling public. <br />8. Vehicles necessary for survey work may be parked in Sate Highway right of way off paved <br />shoulders. Auxiliary support and employee vehicles shall remain outside the right of way. <br />9. Detonating explosive charges within State right of way is prohibited. <br />10. Permittee shall provide for the safe passage of pedestrians. <br />11. Information pertaining to Caltrans survey monumentation may be obtained upon request to: <br />Survey Section, Department of Transportation, P.O. Box 2048 Stockton, CA 95201. <br />Telephone, (209) 948-3983. <br />The State of California, Department of Transportation makes no assurance or expressed warranty <br />that the pians are completed or that the planned construction fits field conditions. Should <br />additional work or modifications of the work be required in order to meet established Department <br />Standards or in order to fit field conditions, the work shall be performed by Permittee as directed <br />by the State's Inspector. <br />5 of 6 <br />
