Any change that would cause the design or operation of the <br /> facility not to conform to the Conditions of this permit <br /> would be considered a significant change and require a <br /> permit revision® <br /> 3® The facility has a permitted daily capacity of 500 tons per <br /> day and shall not receive more than 700 tons per day for <br /> more than 2 days in any week and not more than 15, 000 tons <br /> of waste in any month without first obtaining a revision of <br /> this permit. No limits as to capacity shall apply to <br /> individual categories of waste, but the limits specified <br /> herein shall apply to total waste. <br /> 4. A change in the contracted operator constitutes a change <br /> that would require a modification of this permit. <br /> Provisions : <br /> 1 . This permit is subject to review by the local enforcement <br /> agency, and may be modified, suspended, or revoked, for <br /> sufficient cause after a hearing. <br /> 2. Approved safety helmets and respirators shall be provided <br /> for all employees operating equipment at the site® <br /> 3. All landfill personnel should receive formal training in the <br /> inspection, handling and proper disposal of refuse, <br /> asbestos, ash, contaminated soil , and other special wastes <br /> received® Personnel should be aware of the hazards <br /> associated with the waste, the proper safety precautions to <br /> be taken and the type of safety equipment to be worn® <br /> Training sessions should be held periodically and a record <br /> kept of when each session was given, its content, and <br /> attendees. Signatures of each session attendee should be <br /> obtained as evidence that the session was conducted® <br /> 4. Adequate measures for prompt fire control as required by <br /> local fire authorities shall be maintained by the operator. <br /> 5. Adequate measures to minimize the creation of dust at the <br /> disposal site shall be maintained -by the operator® <br /> 6. Adequate litter control equipment shall be maintained around <br /> the working face of the site. <br />
