0 0 <br />66270.30, Permit Conditions. <br />a. On October 21, 1990, Will Rowe and Stephen Posner <br />of the DTSC observed the following deviations from <br />the SAP: <br />i. Dry Wells <br />Monitoring Wells 1, 3, and 12, required to be <br />sampled in the SAP, were dry. These wells <br />were reported dry in the First Quarter 1991 <br />Monitoring Report and were replaced prior to <br />the First Quarter 1991. Monitoring Report by <br />wells 1B, 3B, and 12B. <br />This deviates from procedure stipulated in the <br />SAP (Sample -Collection -Measurement of Static <br />' <br />Water Level Elevations). Page two of the SAP <br />stipulates that dry wells be decommissioned. <br />ii. Well Casing Reference Marks <br />There are no survey points on the well casings ` <br />to serve as reference for water level <br />measuring. <br />This deviates from procedure stipulated in the <br />SAP (Sample Collection -Measurement of Static <br />Water Level Elevations'). Page one of the SAP <br />stipulates that the measurement of depth of _ <br />water in wells, ,...will be referenced to the <br />permanent NGVD survey point previously place <br />on the side of the casing." <br />iii. Total Depth Measurements <br />The total depths of the wells were not <br />checked. <br />This deviates from procedure stipulated in the <br />SSP (Sample Co11 lection -Measurement of Stat�.c <br />Water Level Elevations). <br />Page one of the SAP specifies that the depth <br />to the bottom of the well and the depth of <br />standing water in the well shall be measured. <br />b. On October 22, 1991, Will Rowe, Stephen Posner, and <br />Brian Lewis of the DTSC, observed the following <br />deviations from the SAP: <br />i.
