6.1.9 General Falls/Ladders <br /> Observe APTIM Procedure AMS-710-02-PR-00500 and AMS-710-02-GL-00901. In addition, observe the <br /> following general falls/ladders procedures and practices: <br /> • APTIM requires 100%fall protection when working from ladders with a potential fall of 6ft (1.8.m) <br /> or more. The specific requirements for implementation are found in AMS-710-02-PR-00900 (Fall <br /> Protection). <br /> • Use Type 1A rated ladders. <br /> • Make sure ladder rungs are sturdy and free of cracks. <br /> • Use ladders with secure safety feet. <br /> • Pitch ladders at a 4:1 ratio. <br /> • Secure ladders at the top or have another person at the bottom to help stabilize it. <br /> • Ladders used to access an upper landing surface shall extend at least three feet above the upper <br /> landing surface. <br /> • Do not use ladders for access above six feet. Instead, use aerial lift. <br /> • Use non-conductive ladders near electrical wires. <br /> • The top step of a stepladder should not be used as a step. <br /> - Do not carry any object or load that could cause a loss of balance or a fall. <br /> - All ladders need to be inspected using the APTIM ladder Inspection Form (AMS-710-02-FM-00501) <br /> (Appendix A) <br /> 6.1.10 Vehicle Safety Management <br /> Motor vehicle incidents are the number one cause of occupational fatalities. Fifty percent or more of <br /> vehicle safety incidents occur while backing up. All personnel involved in the operation and use of <br /> APTIM and/or rented/leased vehicles will comply with APTIM procedures. APTIM requires employees to <br /> use seat belts at all times when traveling in APTIM owned or leased/rented vehicles. <br /> APTIM personnel are expected to incorporate safe actions and preparations to avoid vehicle accidents <br /> and person injury during work and off-hours. Breaks should be planning into lengthy drives, including <br /> rotation of drivers at regular intervals. If parking areas are busy or crowd and more than one individual <br /> is traveling in the same vehicle, one employee should remain outside the vehicle as it leaves the parking <br /> space to assist the driver with traffic observation. <br /> 6-5 <br />
