6.1.12 Excavation and Trenching <br /> Observe APTIM Procedure AMS-710-02-PR-01600. In addition, observe the following Excavation and <br /> Trenching practices and procedures: <br /> • Ensure a competent person is assigned. <br /> • Before any excavation can be started, an excavation permit(AMS-710-02-FM-01601) (Appendix A) <br /> must be completed by the responsible Superintendent with input from the Authorized Person for <br /> underground testing (utility identification) and the Excavation Competent Person. Appropriate <br /> Client approval is also necessary where required by Client regulations. Client and/or utility <br /> company representatives shall be notified 24 hours prior to beginning excavations. <br /> • Check for utilities (see Underground Utility Line Contact,Appendix A). <br /> • Have a Professional Engineer(PE) evaluate all excavations deeper than 20 feet. <br /> • Excavations shall be inspected by a Competent Person and the results recorded on AMS-710-02- <br /> FM-01602 (Appendix A): <br /> — Prior to entry <br /> — After rain or snowfall <br /> — After freezing and/or thawing temperatures occur <br /> — After any condition that can change the integrity of the soil <br /> • Use protective systems(sloping, shoring, shielding)for entry in trenches over five feet deep. <br /> • Provide for rescue for cave-ins. <br /> • Place spoils a minimum of two feet from edge. <br /> • Monitor excavations over four feet deep for hazardous atmospheres and LEL/oxygen. <br /> • Place ladders within 25 feet of lateral travel if the excavation is over four feet deep. <br /> • Place barriers around excavations near pedestrian access. <br /> - Follow work zone security procedures of Section 6.1. <br /> 6.1.13 Confined Spaces <br /> Employees and contractors will follow permit-required confined space entry standards described in CFR <br /> 1910.146 and AMS-710-02-PR-01700 when confined space entry work is required. <br /> 6-8 <br />
