A confined space means a space that has all of the following conditions present: <br /> • Is large enough, and so configured,that an employee can bodily enter and perform assigned work <br /> • Has limited or restricted means for entry or exit <br /> — Is not designed for continuous employee occupancy. <br /> Permit-required confined space means a confined space that has one or more of the following <br /> characteristics: <br /> • Contains, or has the potential to contain, a hazardous atmosphere <br /> • Contains a material that has the potential for engulfing an entrant <br /> • Has an internal configuration such that an entrant could be trapped or asphyxiated by inwardly <br /> converging walls or by a floor which slopes downward <br /> — Contains any other recognized, serious safety or health hazard. <br /> Entry means the action by which a person (or part of a person) passes through an opening into a permit- <br /> required confined space. Entry includes work activities performed in that space and is considered to <br /> have occurred as soon as any part of the entrant's body breaks the plane of an opening into the space. <br /> Reaching into the space with a tool is not considered entry. <br /> In addition,following confined spaces procedures and practices should be observed: <br /> • A hazard assessment shall be performed and documented on the Safe Work Permit (AMS-710-02- <br /> FM-01701) (Appendix A) prior to entry. The supervisor in charge of the work shall schedule this <br /> action. <br /> • Ensure employees are trained in the hazards of confined spaces. <br /> • Post Confined Space Entry Permits at the entrance to the space. <br /> • Have a copy of the confined space entry procedure available. <br /> • Establish a rescue plan. Evaluate to ensure rescuers are qualified. <br /> • Ensure an entry supervisor is present at each permit-required entry. <br /> — Ensure the required extraction/fall protection devices are being used properly. <br /> 6.1.14 Electric Shock <br /> Observe APTIM Procedure AMS-710-02-PR-01400 and AMS-710-02-PR-01500. In addition, observe the <br /> following procedures and practices to prevent electric shock: <br /> • Maintain appropriate distance from overhead utilities: <br /> 6-9 <br />
