– Replace all guards before start-up. Remove cranks, key, or wrenches used in service
work.
6.1.16 Lockout/Tagout Procedures
Lockout procedure consists of placing a lock on an energy isolating devise to ensure that the device and
the equipment it controls cannot be operated until the lock is removed. The lock can be either the key
or the combination type. If a keyed lock is used, only the person being protected from the energy source
(or providing protection as an authorized attendant for a confined space) should have the key; company
locks for which several individuals may have a key should not be used. Lockout/tagout procedures
should be performed in accordance with APTIM Procedure AMS-710-02-PR-01500. The following items
must be addressed when using lockout procedures:
• Locks must be of such a quality that a significant force would be required to detach them (e.g., bolt
cutters).
• If using group lockout, the specific codes and state requirements should be reviewed and followed.
– All employees must be trained in lockout/tagout procedures.
6.1.17 Physicallnjury/Ergonomics
Review APTIM procedure AMS-710-01-PR-03700. Observe the following procedures and practices to
prevent physical injuries:
• Wear hard hats and safety glasses when on site.
• Maintain visual contact with the equipment operator and wear an orange safety vest when heavy
equipment is used on-site or when adjacent to or in roadways.
• Avoid loose-fitting clothing.
• Prevent slips, trips and falls—keep work area uncluttered.
• Keep your hands away from moving parts(i.e., augers).
– Test the emergency shutoff switch on the drill rig daily.
6.1.18 Vehicular Traffic
Observe the following procedures and practices regarding vehicular traffic:
• Wear traffic safety vest when vehicle hazard exists.
• Use cones, flags, barricades, and caution tape to define work area.
6-11
