SOP-002 Standard Operating Procedure <br />Field Activity Records <br />Page 1 of 6 Revision No. 1 April 2019 <br />Sharpe Army Depot <br />1.0 Introduction <br />1.1 Purpose <br />This Standard Operating Procedure (SOP) establishes criteria to record onsite field activities, including: <br />• Format and content for field logbooks and activity-specific forms <br />• Visual recording using photography or video <br />1.2 Scope <br />This SOP applies to all personnel who record information in field logbooks and on forms, or employ photographic or video techniques to document site activities. <br />1.3 Importance of Documentation <br />An essential part of the sampling/analytical portion of any environmental project is to document all activities properly. Records can include: <br />• Field Logbook (primary) <br />• Field Activity Form (primary) <br />• Visual recordings (photographs or video) <br />Analytical data, or conclusions based upon analytical data, may be used in litigation. Therefore, the history and accountability of all samples must be accurately recorded and available to demonstrate that the data are a true representation of the environment. The records must be an accurate representation of field activities, procedures, and site conditions. The field logbook and forms, or visual recordings may be used as evidence in legal proceedings to defend procedures and techniques employed during site investigations. Therefore, field documentation must be factual, complete, accurate, and consistent. <br />2.0 Equipment List <br />Obtain new field logbooks and activity-specific forms as needed from the Field Manager/Task Leader. The individual using the field logbook will be responsible for its care and maintenance throughout the field task. <br />3.0 Procedures <br />3.1 Field Logbook <br />Field logbooks will be bound with lined, consecutively numbered pages. All pages must be numbered before the initial use of the logbook. The following information will be recorded on the cover, binding, or inside the front cover of the logbook: <br />• Field document control number <br />• Activity <br />• Contractor’s name
