SOP-002 Standard Operating Procedure <br />Field Activity Records <br />Page 2 of 6 Revision No. 1 April 2019 <br />Sharpe Army Depot <br />• Phone number <br />• Site contact (Field Manager/Task Leader) <br />Follow these requirements when using a logbook: <br />• The date must be recorded at the top of each page. <br />• If data collection forms are specified by an activity-specific plan or procedure, the information need not be duplicated in the logbook. <br />• All changes must be made with a single line through the deletion. Changes must be initialed and dated. <br />• A diagonal line must be drawn through any space left at the bottom of each page. <br />• The bottom of each page will be signed by the author. <br />• Do not remove any pages from the logbook. <br />3.2 Field Activity Form <br />Field activity forms are specific to the activity being performed. They include, but are not limited to: <br />• Boring log <br />• Well construction log <br />• Well development log <br />• Groundwater purging and sampling datasheet <br />An example of each of these forms is attached to this SOP. <br />Each field form must contain the following: <br />• Date <br />• Location <br />• Name of the personnel performing the work <br />• Sufficient detail to allow a third party to understand what task was performed and what measurements/data was collected <br />3.2.1 Record Time <br />Entries into field forms and logbooks will be preceded with the time of the observation. The time should be recorded frequently and at the point of events or measurements that are critical to the activity being logged. <br />3.2.2 Record Description <br />At each station where a sample is collected or an observation is made, a detailed description of the location is required. <br />If a map is not already available showing the sample location, a sketch of the location is required. The sketch or diagram should be detailed enough for other individuals to locate the points at future times. A
