Project Hazard Identification, Risk Assessment and Control (HIRAC) Form
24
GENERAL WORK ACTIVITIES
Potential Hazards
Initial Risk
Ranking
(R/Y/G)
Hazard Controls
(including PPE requirements)
Applicable Policy or
Procedures
Residual Risk
Ranking (R/Y/G)
• If symptoms arise – move the worker into the shade, loosen
clothing, wet and fan the skin, place ice-packs in the armpits
and neck. Give the worker something to drink.
• If worker loses consciousness or appears confused or
uncoordinated immediately call for emergency services.
• Be aware of your physical condition and health problems and
that of others.
• Establish and follow a work/rest schedule and hydration
schedule as necessary based on the Heat Index Value (Table
22A-2).
• Develop a plan to reschedule or terminate work if conditions
become too risky.
Extreme weather –
storm conditions

(High wind
conditions, lighting
hazards, snow and
ice hazards, etc.)
Y • For any outside work scheduled when the wind speed
exceeds 40 mph, a Safety Time Out/Task Hazard Assessment
should be held to determine if the work is of such a high
priority that it should be performed in such conditions.
• Consult Table 22C-1 when the National Weather Service
(NWS) issues weather warning/hazard information. Be aware
of potential issues related to severe weather such as flooding,
downed trees or electrical lines, heavy snow or rain which
could impede travel, etc.
• Shut down work and get indoors when lightning is in the area.
Follow the Lightning 30/30 rule:
o If it takes less than 30 seconds to hear thunder after
seeing the flash, lightning is near enough to pose a
threat.
o Wait at least 30 minutes after the last indication of
lightning/thunder to resume work.
• Prepare for potential extreme weather conditions in regards
to proper clothing and travel/vehicle emergency equipment
(snow tires, chains, space blanket, food rations, extra water,
etc.)
HSE Manual, Section 22 –
Extreme Weather Program

Attachment 22C - Storm
Condition Guidelines

G
