Project Hazard Identification, Risk Assessment and Control (HIRAC) Form <br />27 <br />GENERAL WORK ACTIVITIES <br />Potential Hazards <br />Initial Risk <br />Ranking <br />(R/Y/G) <br />Hazard Controls <br />(including PPE requirements) <br />Applicable Policy or <br />Procedures <br />Residual Risk <br />Ranking (R/Y/G) <br />• Set up work areas away from pavement heaves, potholes, <br />railroad tracks, etc. <br />High noise levels Y • Use hearing protection when exposed to excessive noise <br />levels (greater than 85 dBA over an 8-hour work period). <br />• Wear hearing protection if you have difficultly hearing <br />someone talk at a normal volume at a distance of about 3 feet <br />(approximates the 85 dBA action level). <br />• Employees who are exposed to excessive noise on a regular <br />basis should be enrolled in SLR’s Hearing Conservation <br />program (includes annual audiograms and training). <br />HSE Manual, Section 37 – <br />Hearing Conservation <br />Program <br />G <br />Infectious diseases <br />prevalent in your <br />location (COVID-19, <br />general influenza, <br />etc.) <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />R General Guidance: <br />• Prior to mobilizing for the work ensure adequate supplies of <br />sanitizing hand gel or cleaning wipes are available as well as <br />disposable gloves, eye protection and face coverings in case <br />social distancing becomes compromised or otherwise <br />determined by risk. <br />• Check with the client concerning any special site <br />controls/procedures and likely number of people to be on <br />site. <br />• Seek medical attention if experiencing signs/symptoms of <br />COVID-19 illness (fever, cough, shortness of breath and/or <br />any breathing difficulties). <br />• Notify your supervisor if you have had or suspect having <br />COVID-19 before returning to work. Do not return to work <br />until free of symptoms for 14 days. <br />• Notify your supervisor and stay home if you find out that you <br />have been in close contact with a confirmed or suspected <br />COVID-19 case. <br />• Notify your supervisor if you have travelled or recently <br />returned (within the last 14 days) from destinations with a <br />Health Travel Notice issued by the CDC (includes China, Iran, <br />South Korea and Europe). Refer to the CDC website for <br />updated information. <br />Centers for Disease Control <br />and Prevention (CDC) <br />guidance information. <br />Also refer to local health <br />department guidance for <br />region-specific outbreaks. <br />Y
