Project Hazard Identification, Risk Assessment and Control (HIRAC) Form
28
GENERAL WORK ACTIVITIES
Potential Hazards
Initial Risk
Ranking
(R/Y/G)
Hazard Controls
(including PPE requirements)
Applicable Policy or
Procedures
Residual Risk
Ranking (R/Y/G)

Infectious diseases
prevalent in your
location (COVID-19,
general influenza,
etc.)

• Stay home if you are sick, except to get medical care.
• If sick, work with your supervisor concerning options for
working from home.
• Cover coughs and sneezes with a tissue when you cough or
sneeze or use the inside of your elbow.
• Wear a facemask if you are sick and around other people.
Current federal guidelines indicate a cloth (non-surgical) mask
is enough for those without symptoms. Please follow state
guidelines on masks.
• Wash your hands often with soap and water for at least 20
seconds or use a hand sanitizer that contains at least 60%
alcohol.
• Avoid touching your eyes, nose and mouth with unwashed
hands.
• Avoid any physical contact with people (no hand shaking, etc.)
• Maintain a minimum 6-foot distance between yourself and
other people at all times (social distancing).
• Clean you hands and follow appropriate sanitization
procedures after any interactions with potentially
contaminated surfaces, personnel, etc.
• Clean and disinfect frequently touched surfaces daily.
• Take special precaution to follow the above controls if you are
a ‘high risk’ individual – older adults (> 60 years) or if you
have serious chronic medical conditions such as heart disease,
diabetes, or lung disease.

Field Work Guidance:
• Consider all options for conducting work/meetings remotely
before meeting in person. If possible, conduct meetings
remotely (phone, Skype, etc.)
• If having to meet in person arrange meeting rooms, lunch
areas, etc. to allow for social distancing – at least 6 feet
