Project Hazard Identification, Risk Assessment and Control (HIRAC) Form 32 DRILLING ACTIVITIES Potential Hazards Initial Risk Ranking (R/Y/G) Hazard Controls (including PPE requirements) Applicable Policy or Procedures Residual Risk Ranking (R/Y/G) • Mark overhead utilities (e.g., warning tape, flags, etc.) where heavy equipment, or other equipment, has the potential for contact. • Maintain a minimum clearance of 10 feet between the overhead lines and employees and other conductive materials (e.g., mechanical equipment, vehicles, drill rig structures, etc.) • If voltage of the overhead line is unknown, remain at least 20 feet away from the line. • Refer to Table 10-1 for specific clearance distances based upon line voltages. • Utilize a Spotter when it is possible to violate the minimum clearance distance requirements. Pre-plan communications between the Spotter and equipment operators. • If having to perform work within a restricted distance to an overhead line then actions should be taken, such as de- energizing and grounding the line or installation of protective line insulators. Table 10-1: Overhead Utility Clearance Distances Moving and rotating parts and equipment (Struck by and entrainment related injuries) R • Equipment guards shall be in place around any rotating equipment or personnel otherwise protected from this hazard by distance. • At the beginning of the work shift test that drill rig emergency stops are operating properly. • Set up sampling area away from drilling operations. • Drilling in streets, parking lots or other areas of vehicular traffic requires definition of the work zone with cones, warning tape, work vehicles, etc. and compliance with local ordinances.
