Project Hazard Identification, Risk Assessment and Control (HIRAC) Form <br />33 <br />DRILLING ACTIVITIES <br />Potential <br />Hazards <br />Initial Risk <br />Ranking <br />(R/Y/G) <br />Hazard Controls <br />(including PPE requirements) <br />Applicable Policy or <br />Procedures <br />Residual Risk <br />Ranking (R/Y/G) <br />• Wear well-fitting (not loose) clothing; remove jewelry, <br />drawstrings, and other items that may potentially be caught <br />in rotating equipment. <br />• Stay clear of the drill string and other moving components <br />of the drill rig. <br />• SLR employees should never approach the auger string <br />unless the transmission is in neutral or the engine is off, and <br />the augers have stopped rotating. <br />• Move auger cuttings away from the auger with a long- <br />handled shovel or spade; never use hands or feet. <br />• Never clean an auger attached to the drill rig unless the <br />transmission is in neutral or the engine is off, and the auger <br />has stopped rotating. <br />Exposure to site <br />contaminants and <br />preservatives <br /> <br />(Potential for acute <br />or chronic injury <br />from chemical <br />exposure) <br />Y • Pre-plan work activities in conjunction with HSE <br />Management to determine appropriate risk mitigation <br />methods for the determined chemicals-of-concern. <br />• Determine and implement any necessary engineering or <br />administrative controls to minimize worker exposure to <br />hazardous chemicals (e.g., ventilation, dust suppression, <br />work area isolation, etc.) <br />• Coordinate with HSE Management concerning the need for <br />any specific chemical hazard controls – protective clothing, <br />exposure monitoring, medical surveillance, respiratory <br />protection, employee training, etc. <br />• In general, wear disposable chemical-protective gloves and <br />ANSI Z87.1-approved safety glasses.
